ObjectiveThe aim of this study was to explore the characteristic of plasma levels of sCD40, sCD40 L, TNF-α levels in patients with Alzheimer’s Disease and amnestic cognitive impairment; we also explored the relationships between the plasma levels of sCD40, sCD40 L, TNF-α and the cognitive function of Alzheimer’s Disease and amnestic mild cognitive impairment. MethodThere were 25 individuals with Alzheimer’s Disease, 40 with amnestic mild cognitive impairment and 42 with normal control subjects in our study. Neuropsychological assessments Mini Mental State Examination(MMSE), auditory verbal learing test(AVLT), Boston naming test(BNT), Digital span test(DST), Clock Drawing test(CDT), Alzheimer’s Disease Assessment Scale(ADAS), Global Deterioration Scale(GDS), Clinic Dementia Rating(CDR), Activity of daily living(ADL), Neuropsychiatry Inventory(NPI), Hachinski Ischemic Scale(HIS)were tested for estimating cognitive function and dementia severity.The plasma sCD40, sCD40 L, TNF-α were measured by commercial ELISA kits. Results1、For plasma sCD40,no significance difference was observed among AD, aMCI and NC(P>0.05).For plasma sCD40 L,significant higher plasma levels were observed for aMCI compared to AD and NC.No significant difference was observed for AD compared to NC(P>0.05).For plasma TNF-α,significant higher plasma levels were observed for aMCI compared to AD.No significant difference was observed for AD compared to NC and for aMCI compared to NC(all P>0.05).2、For the score of Clock Drawing test(CDT),significant lower score was observed for AD compared to aMCI and NC(P<0.001).No significant difference was observed for aMCI compared to NC(P>0.05).For the total score of Alzheimer’s Disease Assessment Scale(ADAS-cog) and the memory factor of ADAS-cog significant higher score was observed for AD compared to aMCI and NC(P<0.001).Significant higher score was observed for aMCI compared to NC(P<0.001).For the score of the verbal factor of ADAS-cog and the attention factor of ADAS-cog and the operation capacity factor of ADAS-cog and the Neuropsychiatry Inventory(NPI), significant higher score was observed for AD compared to aMCI and NC(P<0.001).No significant difference was observed for aMCI compared to NC(P>0.05).For the score of Boston naming test(BNT) and the Digital span test(DST)forwards and backwords, no significance difference was observed for aMCI compared to NC(P>0.05).3、Spearman correlation analysis show the plasma levels of sCD40 L were significantly negative correlated with the assessment of MMSE(P=0.006) of aMCI. Spearman correlation analysis show there were no correlation among the plasma levels of sCD40, sCD40 L and TNF-ɑ and cognition function of AD. Conclusion1、The plasma s CD40 L may play an importmant role in the pathophysiology of aMCI. And sCD40 L and TNF-ɑ may play an importmant role in the pathophysiology of general decline immune responsiveness in AD.2、The verbal function, the attention function, the operation function and the executive function appear after memory disorder of Alzheimer’s Disease.3、It suggested that sCD40 L may be an independent predictor of cognitive function of aMCI. The plasma sCD40, sCD40 L and TNF-ɑ may not be appropriate for the status indicator of AD.
