Efficacy And Tolerability Of Febuxostat In Hyperuricemic Patients Compared With Allopurinol: A Systematic Review And Meta-analysis

Objective:To systemically review and investigate the eficacy and safety of febuxostat in Hyperuricemic Patients compared with allopurinol.Methods:To search Pub Med、MEDLINE、ISI Web of knowledge、CNKI 、CBM、WANFANGDATA,The search was concluded From September 2004 to September2014.The quality of randomized controlled trials assessing febuxostat and allopurinol for Hyperuricemic Patients was evaluated.And the extracted data were calculated by the Cochrane Collaboration’s software Rev Man 5.0.Result:Seven trials were included. A significantly greater proportion of patients achieved the target serum urate level(s UA <6.0 mg/d L) at the final visit in the febuxostat group compared with the allopurinol groups(69.5% vs 43.6%;OR=3.38;95%CI,1.96-5.81;P<0.01). In subgroup analysis, the proportion of patients who achieved target s UA at the final visit was significantly greater in the febuxostattreated group(40 mg/d) compared with the allopurinol-treated group(100~300 mg/d)(50.6% vs 46.1%;OR=1.22;95%CI,1.02-1.45;P=0.03). As the dosage was increased(80, 120 mg/d), the proportion of patients who achieved target s UA in the febuxostat treated group increased gradually(72.8%, 82.0%, respectively). There was no significant difference in the occurrence of adverse events(AEs) between the febuxostat- and allopurinol-treated groups.Conclusion:Febuxostat was effective in reducing serum urate in hyperuricemic patients with/without gout, and febuxostat(40~120 mg/d) was more efficacious compared with allopurinol(100~300 mg/d). The tolerability of febuxostat for the treatment of hyperuricemia with/without gout is similar to that of allopurinol.
