| Objective:To construct the nursing aid demand index system of county-level hospital, and to develop the questionnaire on the basis of the index system.To explore the actual demand of county-level hospitals and to analyze the related factors.Then,provide experience and guidance for follow-up nursing work which carry out in a pertinence and effectiveness way. Methods:1.Delphi method:On the basis of literature and qualitative research,draft the evaluation index system framework preliminary.In June and July of 2014, invite 27 experts to carry on two rounds consultation.By the positive coefficient of experts to express the concentration degree of expert advice.The expert advice coordination degree is mainly represented by the coefficient of variation and the coordination coefficient.The degree of authority expressed by the expert authority coefficient.The importance of mean evaluation and coefficient of variation are used to modify and add items.2.Pre-survey:According to the needs of nursing aid framework has been constructed and combined with the literature to establish the questionnaire items pool.Then form the last questionnaire by the research group discussion and expert evaluation.In August and September of 2014,250 nurses were randomly selected to attend the pre-survey from in 8 hospitals which belong to County People’s Hospital and the hospital of traditional Chinese Medicine in the 4 counties of Nanchang city.Some statistical methods such as item analysis,factor analysis,correlation analysis and reliability analysis were used to evaluate the reliability and validity of the questionnaire.3.Formal investigation:In October,November,December of 2014,by using random sampling method,6 hospital were randomly selected by using random data table from 4 counties(Nanchang County, Xinjian County, Jinxian county and Anyi county) of Nanchang city. Then,the nurse of 6 selected hospital were as the research object.Here is a total sample of 350 people,and 327 valid dates were obtained.Then by the mean and standard deviation to describe the measurement data.The count data was described by frequency and percentage.T test and single factor analysis of variance were used to compare the difference of the basic data of nurses.Factors of nursing needs in county-level hospital’s statistics speculation were used by Multiple stepwise regression analysis. Results:1.Expert consultation’s results:The positive coefficient of experts was 100%, the number of comments accounted for 62.9%.The two rounds authority coefficient of experts were 0.79, 0.83.The coordination coefficient of two rounds were 0.09-0.16.After chi square test,the difference was statistically significant(P<0.01).It shows some coordination.The final construction of nursing aid demand index system of county hospital includes 3 first class indexes,8 second level indexes,47 third level indexes.2.The results of pre-survey:Exploratory factor analysis results showed that the KMO value is 0.912,the Bartlett test’s r value is 8492.471,it means that reached the significant level, P<0.01.While the spherical hypothesis is rejected, show that the scale is suitable for factor analysis.Factor analysis yielded 6 factors, which explained 53.225% of the total variance.Between each dimension and the correlation coefficient of total scale was 0.532 ~ 0.794.The dimensions of Cronbanch’s α coefficient is 0.789~0.910.The total scale of Cronbanch’s α coefficient and the split half reliability coefficient was 0.923 and0.868 respectively. Eventually removed 3 indexs in level third.Finally,the questionnaire was consist of 3 first level indexes, 8 second level indexes and 44 third level indexes.Reliability and validity of the questionnaire is good.3.The on-site investigation’s results:1).The total demand of nursing aids of country-level hospital in Nanchang city is in a high level(4.29±0.47).Among them,helping content demands(4.35±0.47)is highest,while helping time needs(4.00±0.60) is minimum.2).From the needs of specific projects,helping contents:the highest project of knowledge demand is the prevention, care and other related knowledge of incision infection after operation,its demand rate is up to 99.8%.In technical aspect is bedsore nursing of critically ill patients, it accounts for99.9%.In talent aspect is hierarchical training of young nurses,accounted for 99.8%.While in other aspect is the standardized nursing ward round,training of predictability for nurses.The proportion is 99.7%.Helping forms:in aid hospital,the biggest proportion is 99.7%,which is send nurses to help hospital to take a refresher course.While in accept help hospital,the best way is nurse training,which is account for 99.8%.Helping time:continue to help 1 year accounted for 96.4% in long-time assistance.And continue to help 4-6 months accounted for 98.7% in short-time assistance.3) By multiple step-wise regression analysis,the main influencing factors of nursing aid needs included the work area,the department,professional ranks and titles,the highest degree,whether to accept the aid of nursing and so on.The work area is an important factor of affecting the needs of nursing aids in county-level hospital. Conclusion:The index system constructed in this study is reliable,and the nursing aids questionnaire has good reliability and validity.It can provide experience and guidance for follow-up nursing work which carry out in a pertinence and effectiveness way.The nursing aid demand of country-level hospital in Nanchang city is in a high level.The work area,the department,professional ranks and titles,the highest degree,whether to accept the aid of nursing are the main influencing factors.Therefore,suggest that we must as the actual nursing needs of the hospital for guidance,analysis of influence factors of demand and supply,to implement the nursing aid work,then to improve the overall level of nursing service in County hospital. |
