Objective:To evaluate the influence of recombinant human brain natriuretic peptide(rh BNP) on cardiac function and left ventricular remodeling in high risk patients with early acute myocardial infarction(AMI).Methods:To select eligible 52 cases patients with acute myocardial infarction, All patients were randomly divided into rh BNP group(23 cases) and routine group( 29 cases). Routine group was given routine treatment. rh BNP group intravenous pumping rh BNP3-5 days on the basis of routine therapy. Record the level of NT-pro BNP, left ventricular ejection fraction(LVEF), left ventricular end-diastolic diameter(LVEDd) of two groups of patients at the time of admission to hospital immediately and 1 week after treatment, meanwhile record the occurence of cardiac function change(killip classification and cardiac death) within 1 week.Results:1.During treantment, there was no significant difference on cardiac function change situation between rh BNP group and routine group(P > 0.05). 2.After treatment,the NTpro BNP level of rh BNP group was lower than before treatment(P < 0.05);the NTpro BNP level of routine group was higher than before treatment(P < 0.05).After treatment,the NT- pro BNP level of the routine group was higher than the rh BNP group(P< 0.05). 3.After treatment, the two groups LVEF were higher compared with before treatment(P < 0.01), but there was no significant difference between the two groups(P > 0.05)).4.After treatment,the two groups LVEDd increased compared with before treatment(P < 0.01), but the routine group increased more significantly(P < 0.05).Conclusion:The administratration of rh BNP as early as possible can effectively protect cardiac function and inhibit early left ventricular remodeling in high risk patients with early AMI, but in the aspect of restraining deterioration of clinical cardiac function, there was no significant difference in comparison with routine treatment.
