Clinical Research On Hypercoagulability Of 75 Small Cell Lung Cancer Cases

Objective:In order to provide more safeguards on the hypercoagulable state in small cell lung cancer patients,the laboratory coagulation testing and clinical characteristics of small cell lung cancer patients on hypercoagulability would be investigated.Methods:In this paper,we collected a total of 75 small cell lung cancer patients and 44 healthy persons of of The Military General Hospital of Beijing from JAN 2011 and Nov2013,maked record of basic information including age, gender, clinical stages, anti-tumor treatments, coexisent disease, body condition scores, peripherally inserted central catheter,whether the occurrence of thrombosis of patients.According above coagulation test results,we would decide whether there were hypercoagulable state,the characteristic of hypercoagulability on laboratory blood tests and clinical information were obtained.Results:1.The general information of small cell lung cancer group and healthy control group:44 cases of healthy control group, 34 male persons(77.27%), 10 cases of female persons(22.73%); age 34-62 year old, mean age 47.40 ±8.86 years; 75 cases with small cell lung cancer patients, 59 male persons(78.67%), 16 cases of female persons(21.33%); age41-82 year old, mean age 54.00 ±11.35 years.2.In the healthy control group of 34 males and 10 females, clotting abnormalities were five cases(11.37%).60 cases of 75 cases small lung cancer patients,accounted for about80% showed different degree of clotting abnormalities.Small cell lung cancer group fibrinogen(FIB), D-dimer(DD) levels contrasted with healthy group was outstanding more than others(P value of 0.002, 0.004), prothrombin time(PT), thrombin time(TT)were significantly shorter(P value is 0.012,0.003).3.The characteristics of laboratory coagulation testing on small cell lung cancer patients:Appeared in 75 cases of small cell lung cancer patients,60 cases(80%) had an abnormal levels blood clotting results,with the most index D- dimer, there are 48 cases(64.00%), followed by increased FIB in 33(44.00%), and PLT increased in 11(14.67%).PT and APTT were lower than normal,there were 4 cases(5.33%).There were26 cases(34.67%)patients,whose two blood coagulation test results beyond the range, FIB,D- dimer value has the highest percentage of the number of patients, 19 cases(25.33%).There were 9 cases(12.00%) patients with three coagulation parameters exceed the scope of reference, FIB, D- dimer, elevated PLT, 4 cases(5.33%) patients.4.4 The relationship of characteristics on laboratory coagulation testing and clinical features of small cell lung cance cell cancer patients :In the SCLC patients of different age groups,the patients > 61 years of age compared with patients ≤60 years,had a higher FIB, DD(P values were 0.021,0.007), and a prolonged TT(P values were 0.01); Comparied with the patients in Bureau deadline,FIB and DD in patients of extensive stage were significantly increased(P values were 0.032,0.013), the differences of that were statistically significant(P<0.05);There are significant differences between ECOG scores of different groups,the value of FIB, DD in patients of ECOG> 2groups were higher than that of ECOG≤2 group, the discrepancy was outstanding deviation(P value is 0.023,0.016); PT value of SCLC group with the disease were shorter than those without comorbidities, the difference there was statistically significant(P values were 0.025); There was no significant difference in gender, PICC catheter and treatment, the discrepancy was outstanding deviation(p> 0.05).5.The clinical characteristics of 4 cases Venous thromboembolism small cell lung cancer patients:①the median age was 63 years old,both of them were male.②both of them were on extensive stage.③two cases in which ECOG score greater than 2 points.④two cases were with comorbidities, one case with hypertension alone, while another one with hypertension and diabetes. ⑤one case was receiving chemotherapy, two cases were on radiochemical synchronization.All of the four patients with venous thromboembolism had abnormal coagulation indexes,D-dimer, FIB levels were higher than normal levels in the above patients.Two of them gad a D-dimer levels greater than 2 times the upper limit of normal, two patients PT were shortened, and one patient TT was shorten, but all of the PLT count are in normal levels.Conclusions:The blood of patients with small celllung cancer often have high blood coagulation state(80.00%), with FIB, D-Dcoagulation index rise at more the Elderly(> 60 years),ECOG score more than 2 points, extensive stage, and with concomitant diseases were clinical predisposing factors. We should consider the above risk factors and coagulation characteristics to reduce the incidence of thrombotic adverse events and improve prognosis.
