Objective:To investigate the diagnostic value of the neck artery murmurs in carotid artery disease and subclavian artery disease.Methods:A total of 185 inpatients with cerebral ischemic stroke or transient ischemic attack(TIA). We will take a carotid and subclavian artery auscultation for inpatients in the hospital that day, and then carotid artery ultrasound examination.Last the data was Summarized by the third person.Taking carotid artery ultrasound examination as golden standard,the research object on the basis of severity were divided into normal,plaque group,mild stenosis,moderate stenosis and severe stenosis group. The research object is divided into noise and no noise with carotid auscultation results.The data was statistically analyzed.Results:1.The sensitivity of carotid bruit in diagnosis carotid lesion,carotid plaque carotid stenosis were 51.4%,58.3%,79.2% respectively;The specificity were 86.6%,73.4%,63.2%,respectively;The false negative rate were 48.6%,41.7%,20.8%,respectively;The false positive rate were 13.4%,26.7%,36.8%,respectively;The positive likelihood ratio were3.84,2.18,2.15,respectively;The negative likelihood ratio were 0.56,0.57,0.33,respectivel-y;The positive predictive value were 86.6%,73.4%,63.2%,respectively;The negative predictive were 86.6%,73.4%,63.2%,respectively;The youden index were 0.38,0.317,0.424,respectively;The diagnosis odds ratio were 6.84,3.86,6.53,respectively;The kappa value were 0.42、0.30、0.12,respectively.2.The sensitivity of subclavian artery bruit in diagnosis subclavian artery lesion、subclavian artery plaque,subclavian artery stenosis were 66.7%,64%,76.5%,respectively;The specificity were 54.1%,54%,50.7%,respectively;The false negative rate were33.3%,35.9%,23.5%,respectively;The false positive rate were 13.4%,26.7%,36.8%,respe-ctively;The positive likelihood ratio were 1.45,1.39,1.55,respectively;The negative likelihood ratio were 0.62,1.28,0.46,respectively;The positive predictive value were29.3%,23.9%,7.1%,respectively;The negative predictive were 85%,86.9%,97.8%,respecti-vely;The youden index were 0.21,0.18,0.272,respectively;The diagnosis odds ratio were2.35,2.1,3.35,respectively;The kappa value were 0.28,0.10,0.05,respectively.Conclusions:1.The Carotid artery auscultation can serve as a method of the diagnosis of carotid lesions,especially the preliminary screening of carotid plaques and stenosis,but not a substitute for carotid artery color doppler ultrasound.2.We should be more cautious in the diagnosis of subclavian artery subclavian artery lesions, plaque and stenosis when adopting the subclavian artery murmurs.
