BackgroundAs the increasing awareness of physical examination, widely use of prostate specific antigen(PSA) and improvement of the prostate biopsy technique in our country, more and more of asymptomatic and relatively young patients with localized prostate cancer was diagnosed. Currently, with the improvement of laparoscopic surgical instruments and surgical techniques, many doctors use Laparoscopic radical prostatectomy(LRP) to treat patients of localized prostate cancer with more than 10 years of life expectancy, the clinical outcome is remarkable. However, the complication of postoperative incontinence happened in some patients. ObjectiveThe aim of this study is to observe the recovery of urinary control after Laparoscopic radical prostatectomy, and discuss the factors associated with urinary incontinence after laparoscopic radical prostatectomy. Materials and methodsFrom October 2012 to October 2013, 78 patients with prostate cancer after laparoscopic radical prostatectomy were analyzed retrospectively, were also asked about continence in one year. Then the clinical data was recorded. Risk factors associated with incontinence in these patients were analyzed by univariate analysis and coxanalyses. All data was analyzed with SPSS17.0 statistical software. Result1.The postoperative continence recovery rate of 1、1-3、3-6 were 25.64%、56.41%、15.38% respectively, and all patients had normal micturition after 8 months.2.Unicariate factors analysis revealed the age(P=0.000), prostate volume(P=0.019), TURP(P=0.011), diabetes(P=0.008), bladder neck support(P=0.001), time of operation(P=0.020)pathological positive margin(P=0.023) and transfusion(P=0.000) were proven significant when univariate analysis;but the other BMI(P=0.350),PSA(P=0.742), preoperative endocrine(P=0.138),pathological stages(P=0.092) pathological stages(P=0.092) were not.3. Multiple factor analysis revealed that age(P=0.000), prostate volume(P=0.009) and TURP(P=0.009) were the main factor for continence. ConclusionMost of patients received good outcome in urinary continence at 4 weeks to12 weeks after laparoscopic radical prostatectomy, and in these patients urinary continence recover rapidly within 10 weeks. Age, prostate volume and TURP were the main factor for continence.
