Objective To evaluate the sub-health status of community residents and its major risk factors,and discuss the relationship between the mi RNA-125a-5p、mi RNA-155、mi RNA-21 and mi RNA-133 a of peripheral blood mononuclear cells with the sub-health status from the epigenetic factors. To provide a theoretical basis for preventing the sub-health status of community residents, and discussing the epigenetic factors and mechanism of sub-health status. Methods During April to September in 2014, community residents above age 18 years were selected as the research objects from Fu Qing City、Chang Le City and Feng Ze District in Quan Zhou City. The sub-health condition was evaluated by the questionnaire which was made by ourselves. The ROC Curve and Minkowski distance method were used to determine the optimal prediction cut-off point of SHS-30. According to the cut-off point evaluating the incidence and explore the main influence factors of sub-health status in community residents. The expressive discrepancy between the sub-health status group and healthy controls were compared by quantitative real-time PCR in the mi RNA-125a-5p、mi RNA-155、mi RNA-21 and mi RNA-133 a of peripheral blood mononuclear cells. Result(1)6007 residents were selected and 5275 valid questionnaires were finished, and the effective rate was 87.81%. SHS-30 is a reliable, valid and sensitive measurement scale for measuring the sub-health of community residents.(2)The areas under ROC curves was 0.826, The standard error of the area was 0.013, the 95%CI was 0.800~0.852. Comparing the area with 0.5 which was served as a reference of a test with no diagnostic value, the difference was all significant(P<0.05).(3)Minkowski Distance was used to discuss the SHS-30’s appropriate criteria margins. When the diagnosis was 7.5 points, the sensitivity was 0.863, it was the discriminant criteria of sub-health status.(4)Surveyed the crowd, the sub-health status were account for 39.1%. The incidence of sub-health status were significantly higher for female or more than 60 age group. With regard to marital status, population of separating marriage had the highest incidence rate of sub-health. The incidence rate of sub-health in the population with high school education levels or primary education was higher than junior school education. These differences had statistic significance(all P<0.001). As shown in the result of influence factor analysis, the lack of health literacy, smoking, irregular diet and high BMI were the main risk factors for residents.(5)The expression levels of mi R-125a-5p and mi RNA-155 in sub-health group were higher obviously than those of healthy control group, but the expression level of mi R-133 a was lower than healthy control group, the differences were all significant(P<0.05). Conclusion(1) The rate of sub-health status was 39.1% in the community residents, the sub-health status was the result of joint action of many factors, the lack of health literacy, smoking, irregular diet and high BMI were the main risk factors for residents. We should enhance the health literacy and strengthen the education of healthy lifestyle in the community residents.(2) Comparing the healthy control group with sub-health group, the level differences of mi R-125a-5p、-155、-133 a were all significant, they might become mi R-125a-5p、mi R-155、mi R-133 a may become the biomarkers of sub-health status, also could provide reference basis for the mechanism study.
