Objective: The aim of the current study is to investigate the prognostic factors of periampullary carcinoma underwent radical pancreaticoduodenectomy by multivariate COX model analysis methodMethods: We reviewed the records of 87 patients who underwent radical pancreaticoduodenectomy for periampullary carcinoma between January 2009 to April 2014 in the First Affiliated hospital of Fujian Medical University, the date of outcome/survival were analyzed retrospectively,groups of Ampullary carcinoma and Pancreatic Head Carcinoma were set for The Control Analysis. Kaplan–Meier survival curves were created and compared by log-rank analysis. Multivariate analysis was undertaken using Cox proportional-hazards method.Results: There were 87 patients in the whole groups, including 54 cases of Ampullary Carcinoma group(62.1%), 33 cases of Pancreatic head carcinoma group(37.9%).The median survival of Ampullary carcinoma is 36 months, and Pancreatic Head Carcinoma is 16 months. There was significant difference between the two groups(p=0.005). Univarite analysis showed that the related prognostic risk factors after Pancreaticoduodenectomy for Periampullary Carcinoma associated with site of origin(p=0.005),age(p=0.001),T stage( p=0.009),surgical margin(p=0.010),nodal metastasis(p=0.015), neural invasion(p=0.009). Multivariate analysis showed that the independent prognostic risk factors after Pancreaticoduodenectomy for Periampullary Carcinoma associated with nodal metastasis(p=0.033),age(p=0.046),T stage(p=0.010)。Conclusions: Prognosis of Ampullary carcinoma was better than Pancreatic HeadCarcinoma after Pancreaticoduodenectomy. Site of Pancreatic head, age ≥60,T3-T4 stage,positive surgical margin, nodal metastasis, neural invasion are the related prognostic risk factors associated with poor prognosis after Pancreaticoduodenectomy for Periampullary Carcinoma. Nodal metastasis, age≥60 and T3-T4 stage are the independent prognostic risk factors associated with poor prognosis after Pancreaticoduodenectomy for Periampullary Carcinoma.
