| Objective:To compare the therapeutic effect of desogestrel ethinylestradiol tablets (Marvelon) and estradiol valerate/estradiol cyproterone tablets (Climen) on endometrial repair,menstrual cycle,and body weight change in patients with missed abortion after uterine curettage.Methods:From December of 2012 to May of 2014 in Department of Obstetrics and Gynecology, the First Affiliated Hospital of Chongqing Medical University and University-Town Hospital of Chongqing Medical University,98 patients with missed abortion after uterine curettage were divided randomly into Marvelon group (n=27), Climen group (n=25),and control group(n=30 Respectively. All patients took mifepristone tablets and misoprostol tablets for drug abortion before uterine curettage, and also received cephalosporin to prevent infection and Leonuris for uterine restoration after uterine curettage. Patients in Marvelon group and Climen group took 1 tablet of Marvelon/day or 1 tablet of Climen/day at the same day when uterine curettage was performed. All the drugs were stopped at the 21th day for one cycle.The next cycle began at the first day of menstruation, and the treatment lasted for 3 cycles.Condom was used by patients in Climen group and control group for contraception. The time of vaginal bleeding, endometrial thickness at the 14th day,and body weight on the 3th month after uterine curettage were observed.Results:The time of vaginal bleeding after uterine curettage was 2.39± 0.63 days in Marvelon group,3-72±0.74 days in Climen group,and 7.37± 1.59 days in control group respectively. The vaginal bleeding time was shorter in in Marvelon group than in Climen group (p<0.01),and it was shorter in both Marvelon group and Climen group than in control group (p <0.01).The time of menstruation restoration was 24.48±1.60 days in Marvelon group,25.44±2.06 days in Climen group, and 33.43±3.83 days in control group respectively. The time of menstruation restoration was shorter in both Marvelon group and Climen group than in control group (p <0.01),however, it was not significantly different between Marvelon group and Climen group(p>0.01).Endometrial thickness at the 14th day after uterine curettage was 10.30±1.14mm in Marvelon group,12.96± 0.99mmin Climen group and 10.40±1.57mm in control group.Endometrial thickness at the 21th day was thicker in Climen group than in Marvelon group (p<0.01)..nevertheless, it was not significantly different between Marvelon group and control group (p>0.01).Body weight change on the 3th months after uterine curettage was 0.75+0.62kg in Marvelon group,0.78±0.59kg in Climen group, and 0.76±.59kg in control group respectively. There were no statistical difference of body weight change among the three groups.Conclusion:Taking short-effect oral contraceptive timely or sequential treatment of estrogen and progestin had significant effects on endometrial repair, reduction of vaginal bleeding and restoration of menstrual cycle in patients with missed abortion after uterine curettage, however, body weight was not affected remarkably during a short period. |
