Tumefactive demyelinating lesions (TDLs) is a mass-like central nervous system inflammatory demyelinating disease, which have obvious mass effect and perilesional edema but mild manifestations. A majority of TDLs occur in the cerebra, and a few are found in the spinal cord. Patients of TDLs had a variety of clinical manifestations, such as dyskinesia, dysesthesia, and cerebellar dysfunction. It is difficult to distinguish from neoplasm, especially glioma and primary central nervous system lymphoma for TDLs. In recent years, generous contrast study had been done between TDLs and neoplasm. Although the application of magnetic resonance spectroscopy (MRS) and diffusion tensor imaging (DTI) contribute to diagnosis, the histological examination is still a golden criteria. The major therapy is corticosteroids, but immunosuppressor and plasma exchange may also be effective for TDLs. Prognosis of tumefavtive demyelinating lesions is relative good, but the risk for translating into multiple sclerosis increases.
