Recently, due to an aging population and prevalence of cardiovascular risk factors, the chronic disease elderly who suffer from Cardiovascular diseases which is mainly the infarction, stroke, coronary heart disease, etc.), as well as hypertension and diabetes still have a high morbidity and mortality rates, and relevant studies show that the elderly has led to a large number of deaths and disability if timely rescue has not been given after falls due to the most proper treatment delay, and the importance of pre-hospital aid which is viewed as an important part in public health and the primary aspects of emergency medical services get more and more people’s attention. The traditional Chinese medical emergency system (EMS) mode has a low efficiency because of step by step, with the living standard and public’s raising awareness, and under the requirements of the new health care reform and the new situation of the wisdom of the medical, the application model of pre-hospital, with the combination of the rapid development of intelligent terminals, GPS, mobile communications and other advanced technologies, can greatly increase aid efficiency and reduce disability and mortality to provide people with more timely, more accurate and more efficient emergency services.In this paper, the research and design of the pre-hospital aid and care system based on mobile network for the elderly chronic diseases is proposed, which is based on the research situation of a hospital emergency of Guangzhou and the status of pre-hospital emergency mode at home and abroad through reviewing the relevant literature analyzes, as well as combing with the development trend of the mobile communication network technology and intelligent terminals under the new situation of wisdom medical. It is smart, convenient, effective and capable of allocating and optimizing rational medical resources to improve aid efficiency.This research is based on the Guang dong Provincial Science and Technology Project named "the research and demonstration of pre-hospital aid applied for cardiovascular based on cloud platform" (NO:2013B010401014). The project built a pre-hospital emergency platform to extending the radius of emergency services, reduce emergency response time and improve pre-hospital emergency networks, improve the efficiency of aid work and standardized emergency management through the research of pre-hospital emergency management mode, which is for community hospitals and the majority of the elderly chronic diseases relying on large hospitals, with the application of cloud computing technology, wireless video communication technology, mobile medical technology, health sensor technology and information integration technology.The purpose of this paper is to change the traditional hospital-based pre-hospital emergency mode, but to provide "active" pre-hospital emergency and care services for the elderly chronic diseases, which has the following advantages:Firstly, providing information about the patient’s vital signs to health care through intelligent terminals and mobile communication network technology; Secondly, facilitating health care services staff to fully grasp and understand the past medical and health conditions of the patient through access to existing personal health records or electronic health records;Thirdly, providing more valuable basic information for emergency medical program through real-time monitoring from the car video, and great convenience for remote hospital emergency admissions guide and prepare well in advance, in order to achieve seamless between pre-hospital and hospital better, and to maximize the efficiency of the rescue and to enhance medical resources efficiency. Furthermore, there are the following meanings through achieving this study:(1) To achieve "active", "closed-loop" aid and care for the elderly chronic diseases;The research and development designed pre-hospital aid and care system for elderly chronic diseases based on mobile networks can change the traditional pre-hospital care model, with the application of personal health devices such as blood pressure monitor signs and falling alarm to fall alarms, the smart phone APP (Application) is used to call SOS and to receive monitored signs data and fall alarms, then uploaded to the back-end server via Bluetooth, WIFI and mobile network synchronization, with the purpose of providing patient information for health care personnel conveniently and timely intervention when fallen alarm from patients, and achieving the closed loop aid and care among pre-hospital and hospital via access to patients’ existing electronic health records or personal health records for continuous health care, which greatly improves the efficiency of the pre-hospital aid and care for the patients.(2) To facilitate sharing of patient history and other relevant information through the basic of a many points to point sharing model about electronic health records;With the aim of addressing the "point" inter-operable issues between regional hospitals and community hospitals via establishing a electronic health records platform of many-points to point mode, the electronic health records and the relevant files are uploaded to the platform, and then get when needed, which can realize the sharing of patient-related information, and break the bottleneck of information sharing and avoid information island, and truly become "live" electronic health records, besides, facilitate the first time to obtain health care medical records of and medical history patients in order to provide targeted emergency rescue and grasp time better.(3) To achieve remote real-time monitoring and improve pre-hospital emergency efficiency and treatment rate with the combination of the mobile network technology, video surveillance technology and emergency medical technology;With advanced mobile network technology, hospital can acquire real-time observation of admissions scene, surrounding circumstances and patient’s condition so that surgery and the opening of the green channel and other admissions can be prepared, which can improve aid efficiency, shorten emergency time, reduce intermediate links to provide the most satisfactory emergency service for patients.(4) To achieve the storage,check out and re-use of the patients’ monitoring signs data pre-hospital.The patients can not only store monitored signs data of pre-hospital locally by phone APP,but also upload to the server, Which can facilitate patient access to their own understanding and sign parameters, while can help doctors make more accurate diagnoses proposed a more reasonable solution in patients pre-hospital and referral. Besides, relevant work of the elderly chronic data when mined and analyzed can be served.In this paper, the research and design of pre-hospital aid and care system mode foe elderly chronic diseases mainly involves the following aspects:(1) The research of the IEEE 11073 PHD international communication standard to achieve inter-operable between personal health devices and intelligent terminals;In this paper, the data communication standard protocols between the personal health monitoring devices and intelligent terminal has been studied. the Bluetooth-based paired and connection between the personal health monitoring devices and android-based intelligent terminal has been achieved, which is the most important part when uploaded to the intelligent terminals. And found that the development cycles can be more shortened with the use of international standards than the proprietary protocol when achieve inter-operable between personal health devices and intelligent terminals. Signs data can display on the intelligent terminals via Bluetooth, Then achieve to monitor for the patients’pre-hospital signs condition.(2) To establish a warning model when fall and monitored abnormal signs for the elderly chronic diseases;The fall alarm can emit a timely warning for the elderly patients who fall at home or outdoors with wearing the fall alarm or the smart phone with the function of fall alarm, and notice to guardians by SMS, and synchronized to the back-end server, and if there are any high-risk exception warnings of pre-hospital signs parameters, which can be saved locally on mobile phone and to the server synchronization, in order to facilitate the timely intervention for medical personnel under unusual circumstances, and make more timely and effective action to pre-hospital aid and care.(3)To position the patients’location for aid and real-time monitoring the patients’ status with the use of GPS (Global Positioning System), Video communication and mobile network technology;Patients can aid through a key function of APP on the intelligent terminal for help, upload the specific aid location, use in-vehicle wireless video communications for remote real-time monitoring, so as to remote guidance under emergency condition, transform to the patient-centered active pre-hospital aid model from the traditional hospital-based center model.(4) To establish a sharing model of electronic health records.In order to change many-points to point-to-point model of the shared electronic health records, the back-end data center build an integrated platform to facilitate to upload and share electronic health records between inter-hospital and community, which facilitates the health care to grasp the past medical history of patients when they are under aid condition, and develop more accurate rescue project, and thus make aid more targeted, and conducive to evaluate the patient’s condition and hospital admissions ahead, as well as improve convergence between pre-hospital and hospital.The research of the pre-hospital aid and care model for the elderly chronic diseases based on mobile networks in this paper, which has the following innovations:(1)To establish a sharing model of electronic health records with a combination of pre-hospital aid, then improve the efficiency of aid;Research and design of electronic health records shared model on pre-hospital emergency, patients can get quick emergency response, the doctors have access to the patient’s previous signs and visits information, which can provide the accurate emergency plan and emergency measures prepared in advance with a scientific basis.(2) Signs such ECG parameters and fall alarms are communicated with the use of wireless networks for remote monitoring, and to achieve real-time monitoring the patient’s condition via wireless video communication;With the use of mobile communication network technology, such as 3G, signs of pre-hospital care and remote video communication are achieved. There will alarm when elderly patients fall, and notify guardians and hospitals by SMS, so as to facilitate timely intervention, and the patient’s vital signs parameters can be real-time monitored remotely and emergency guidance can be acquired by monitors and wireless video communication on the first scene and transit, which is avail to assess the patient’s condition, disease research and analysis as well as prepared hospital admissions in order to provide more comprehensive and reliable basis;(3)To make pre-hospital emergency process more intelligent and information, and standardize the convergence work of pre-hospital emergency management.By making pre-hospital emergency process more intelligent and information, with every aspect of the pre-hospital care in-depth, the low efficiency of traditional manual work in aid of convergence, error rate, and low security are solved. and to make pre-hospital emergency management more evidenced, more science, more real-time and more standardized, through data and information records of pre-hospital emergency process and patient’s information.This paper can be divided into following five chapters:Chapter 1 Introduction It introduces the research background and significance of the pre-hospital aid and care system based on mobile network for elderly chronic diseases,and the pre-hospital study abroad situation and development trend are discussed in detail, meanwhile, the origin, content and structure of the paper are outlined; Chapter 2 The system-related technology This chapter overviews the development trend of mobile communication network and describes the Maps API and wireless video communications applied in pre-hospital aid and care system; Chapter 3 System analysis and design The traditional pre-hospital emergency procedures for elderly chronic diseases has been analyzed in this section, and which is transformed, as well as the system demands, and the warning signs and fall alarm model are introduced in detail, And then system architecture, functional design are introduced; Chapter 4 Mobile client and function realization It describes the the main function of the APP and the transceiver process of the monitored data signs and access to the process, besides,IEEE11073PHD (Personal Health Device) international communications standard which achieve inter-operable between personal health devices and smart mobile terminals, as well as Maps API to realize emergency applications on the smart phone are described in detail, and client interface displays a brief description; Chapter 5 Summary and Outlook All this thesis are given a summary and outlook, including conclusion of this stage of study and the prospect of the pre-hospital aid and care for elderly chronic diseases.Currently, the pre-hospital aid and care system functions for elderly, which is based on mobile network has been finished, it can position accurately and upload the aid position of the patient, and has fall warning and signs warning when at a high-risk condition,and conducive to the timely intervention for elderly chronic diseases furthermore, it can reduce pre-hospital time for the elderly chronic diseases effectively, and improving the efficiency of aid greatly. All in all, the design of overall system is reasonable, but specific functions need to be further improved.
