Objective: The study is to investigate the appropriate operation time,and therapeutic effects resulted from different surgical approaches of cauda equina syndrome(CES).Methods: By retrospectively analyzing 27 patient that underwent CES operation with complete follow-up data, the clinical symptoms, operative approaches and postoperative follow-up results of the 27 cases were analyzed comprehensively.Results: There is no significant differences existing in different clinical types. Patients within 2w received a greater therapeutic effect from surgery than the group more than 2w. Surgical outcome shows the patients that underwent “all lamina cutting” received better rehabilitation than those underwent“intervertebral space” or “hemi-1amina cutting”. And the surgical outcome shows no significant differences on the therapeutic effects between the patients with internal fixation and the patients without internal fixation.Conclusion: Delayed diagnosis and treatment on CES would cause serious adverse consequences.With the aim of decompressing and releasing adhesion, operation should be the primary choice of treatments on CES. Timely surgery is an important measure to improve the therapeutic effects. Although internal fixation is not necessary to a better postoperative effect, it can enhance the postoperative stability.
