Objective : To discuss the efficacy and factors of surgery of cauda equina injury with lumbar fracture.Methods:A retrospective randomized study was performed on 29 cases who had undergone posterior stabilization with pedicle screw instrumentation from December 2012 to January 2016 and all patients made a definite diagnosis of cauda equina with lumbar fracture,and 26 cases were identified and invited to follow-up eventually.The patients were numbered 1 to 26,to analyze the recovery efficacy whether it had a correlation with different timing of surgery and the severity of neurologic injury.Results : The cases were followed-up 0.25-3 years.The majority of patients had different degree recovery of cauda equina after surgical decompression.Three of these patients had no significance difference in overall quality of life who were decompressed more than 1 week from symptom onset,and the mean timing of poor outcome group was 8.3 days;Eight of these patients had severe neurologic injury from onset to surgery,and still presented with motor weakness and neurological dysfunction.Conclusions:Consensus exists that the most appropriate treatment ofcauda equina injury with lumbar fracture is surgical decompression,and the severity injury to cauda equina is closely related to postoperative recovery;The timing of surgery may have an direct effect on the recovery after surgery.It is my opinion that surgery should be performed in a timely manner within 7 days of the onset.
