Objective: To investigate the risk factors and epidemic status of central obesity and to provide scientific evidence for early detection and prevention of central obesity.Methods: This research adopts the method of cohort study. the Dehui city of Jilin Province was selected as the center of epidemiological survey of obesity considering the relevant factors such as region, economy and population structure.Multistage(stratified, systematic) sampling method is used to extract samples.In 2007,1200 people participated in the examination and there are actually 918 people in the group eliminating the people whose information is incomplete, or liver and kidney function severely impaired.Follow-up examinations were performed at the 918 people.In 2013, 734 people were followed up, and 178 people who were found to have central obesity at the first diagnosis were excluded from the group.556 people who did not occur central obesity at the first diagnosis were included in the cohort and the outcome events of central obesity with different sex observe were observed.The survey was divided into questionnaire(diet, drinking, smoking, exercise and other daily life style, occupation, education, marriage, ethnic and other basic information and family history), physical examination(height, weight, waist circumference, etc.) and blood examination( liver function,renal function, etc.). All the data were statistically analyzed by SPSS17.0 software.Chi square test was used for counting data and the difference was statistically significant when p<0.05.Non parametric test analysis was used for measurement data using the median, four quantile, and other indicators.Multivariate Logistic regression analysis was used to analyze the risk factors of central obesity.Calculate OR and its 95% confidence interval and compare cohort groups to calculated RR values.Results: 1.The demographic characteristics of research group enrolled in 2007 : The research of this study included 918 subjects in 2007, of which 408 were male and 510 were female. Age at 41~50 and 51~60 years of age accounted for the largest proportion, respectively, 32% and 27.1%,and 18~30 year olds accounted for the smallest proportion, 5.9%. 2.The prevalence rates of central obesity of research group enrolled in 2007:At age 47(39,56), the prevalence of central obesity was 39.4%, of which 408 were male.At age 48(39,56), the prevalence of central obesity was 39.4%, of which 510 were female and at age 46(39,55), the prevalence of central obesity was 48.6%.The prevalence of female central obesity was higher than that of males, and the difference was statistically significant(P < 0.05).In addition, the results of this study showed that the incidence of central obesity was related to the occupation, and was not related to the degree of culture.The prevalence of central obesity in different occupations from high to low is: individual operators, workers, cadres, enterprises and institutions managers, other occupations, farmers, science and education personnel and the difference is statistically significant(P< 0.05).With the increase of BMI,central obesity prevalence rate assumes the trend of escalation.The prevalence of central obesity was respectively 8.8%, 36.3%, 71.9%, 95.2% from normal BMI to severe obesity and the difference was statistically significant(P < 0.05). 3.The results of this study showed that the prevalence of central obesity was 43.3% in 2013 and the cumulative incidence rate of 6 years was 15.3% in the cohort.Cohort of gender and BMI stratified analysis as follows: BMI greater than or equal to 25 kg / m2 group, the incidence of obesity in male was 31.3%, and the incidence of obesity in female was 60.9%.The incidence of central obesity in female group was higher than that in male group, and there was a significant difference in incidence between groups(P < 0.05), RR=1.95. BMI < 25 kg / m2, the incidence of obesity in male was 9.3%, and the incidence of obesity in female was 35.1%. The incidence of central obesity in female group was higher than that in male group, and there was a significant difference in incidence between groups(P < 0.05), RR=3.77.Conclusion: 1.The prevalence of obesity in adults was high in Dehui.In 2007,the prevalence of central obesity was 39.4%.The prevalence of central obesity was 43.3% in 2013. 2.The results of this study showed that the cumulative incidence rate of 6 years was 15.3% in the cohort in Dehui. 3.The results of this study showed that the incidence of obesity in adults in Dehui is related to sex and BMI. 4.When the adult BMI is not less than 25kg/m2, the risk of central obesity in adult women is 1.95 times that of adult males in Dehui.When the adult BMI is less than 25kg/m2, the risk of central obesity in adult women is 3.77 times that of adult males in Dehui.
