Aim:Clinical analysis of the treatment of femoral intertrochanteric fracture with biological prosthesis and bone cement prosthesis。Methods:148 cases of femoral intertrochanteric fracture in department of orthopedics of the Second Hospital affiliated with Jilin University were selected from April 2013 to March 2016, and ages from 29 to 91 years old(mean 78.70 + 8.75). All of cases were divided into two groups by the surgical methods, one group was biological prosthesis group with 77 cases and another group was bone cement group with 71 cases. Two groups of patients were treated with artificial hip joint replacement. Intertrochanteric fracture fixation integrity, fragments of bone defect repair at the femoral neck were required, and incision length, operative time, intraoperative blood loss and intraoperative drainage were recorded during the operations. The routine treatment was used after operation, and postoperative hospitalization days, bed time, weight-bearing exercise time, early and total number of complications and morbidity were all observed.The thigh pain, Harris scores and the number of cases according to the annual were compared in the two groups. Finally clinical effect of the treatment of intertrochanteric fracture with bone cement and biological prosthesis were comparative analysis.Results:Whether it is a biological prosthesis or bone cement pedestrians hip arthroplasty, both in the incision length, operative time, bleeding volume, drainage volume, postoperative hospital staywere all no significant difference(P> 0.05).The total complication rate was 11.69% in biological prosthesis group and 14.08% in bone cement group, there was no significant difference in both total complications of the two groups(P>0.05), but bone cement group were occurred in 2 death cases.There was no difference between the bone cement group and the biological prosthesis group in the postoperative bed time, weight bearing time and pain score(P> 0.05).There was no difference between the bone cement group and the biological prosthesis group in Harris score from preoperative to 6 months after operation(P>0.05), and Harris score of bone cement groupin 1 years and 2 years after surgery was significantly lower than that of the biological prosthesis group(P< 0.05).Conclusion:It is an effective method to treatment of femoral intertrochanteric fracture using biological prosthesis or bone cement type prosthesis in the operation of hip replacement. Although treatment of patients withbone cement or biological prosthesis are all to have agood effect in early ambulation, relieve pain and restore hip function, but the biological prosthesis have a better long-term effect than the bone cement and the risk of death in biological prosthesis was significantly lower than that of bone cement. Biological prosthesis of hip replacement in treatment of femoral intertrochanteric fracture is the future trend.
