Association Between C-reactive Protein, Procalcitonin, Perinuclear Antineutrophil Cytoplasmic Antibodies And Inflammatory Bowel Disease

Objective: To evaluate the clinical significance of detecting c-reactive protein(CRP),procalcitonin(PCT) and perinuclear antineutrophil cytoplasmic antibodies(pANCA) in inflammatory bowel disease(IBD). Methods: A total of 67 patients with IBD were recruited in our study,including 45 patients with ulcerative colitis(UC),22 patients with crohn’s disease(CD),and 20 healthy volunteers as healthy control group.CRP and PCT were detected by immunochromatographic assay,pANCA was detected by enzyme-linked immunosorbent assay(ELISA),to compare the difference of serum CRP,PCT and pANCA among UC,CD and healthy control group.To evaluate the relationship between CRP,PCT,pANCA and the activities of UC and CD. To understand the diagnostic value by combined detection of all the targets for IBD. Results: There were significant differences of serum CRP, PCT and pANCA levels in UC group, CD group and healthy control group,all P<0.05. The level of serum CRP in CD group was higher than that in UC group, the difference was statistically significant. The level of serum PCT made no significant difference between CD group and UC group, P=0.463>0.05.The positive rate of pANCA in UC group( 53.3%) was remarkably higher than that in CD group( 18.2%), P=0.013<0.0167,the difference was marked. The level of CRP and PCT were positively related with IBD severity,p ANCA could predict the activities of UC,and cause little diagnostic value to CD. Combined testing of CRP, PCT and pANCA could improve the sensitivity of diagnosis.Conclusions: Levels of serum CRP, PCT, pANCA in IBD and healthy controls group exist significant differences,they have great significance in diagnosis and differential diagnosis for UC and CD. CRP and PCT relate to the activity of IBD,pANCA could predict activity of UC, pANCA positive rate is much higher in patients with left semi colon of UC.
