Objective: To discuss the clinical value of anti neutrophil cytoplasm antibodies combined serum homocysteine detection for diagnosis of ulcerative colitis,in order to provides the theory basis for clinical. Methods: Based on the retrospective analysis of patients- with digestive department cases, collected in March, 2010- November 2015, 52 cases of ulcerative colitis cases, 48 cases of digestive tract diseases except for ulcerative colitis, and in the same period in our hospital medical check-up the clinical data of 41 healthy people, three groups of subjects was detected in pANCA and Hcy, two indicators are calculated separately, positive rate of single detection and the positive rate of joint detection, sensitivity, specific, positive predictive value, negative predictive value, etc. Results: in the blood of Uc patients, pANCA and Hcy’s positive rate were significantly higher than that of disease control and health control group, compare the difference was statistically significant(P < 0.05), there was no statistically significant difference between disease control group and healthy controls(P > 0.05). pANCA diagnostic sensitivity in patients with UC was 68.91%, the Specific degrees was 97.75%; The sensitivity of Hcy in UC patients diagnosed was 75.00%, the Specific degrees was 91.01%; the sensitivity of joint detection was, the Specific degrees was 91.01%. AS the concentration of Hcy there were no statistically significant difference during the different degree of UC disease. The Hcy level of UC patients with Lesion located in the sigmoid colon and rectum was significantly higher in patients with lesions located in the left half colon and whole colon group patients. pANCA and Hcy joint detection positive predictive value was 84.90%,negative predictive value was 92.05%.Conclusion:There was no obvious correlationbetween Hcy level and the degree of the UC illness, but diseased region was related with Hcy level. The specific degree of pANCA diagnosis in patients with UC is higher, but because of it could appears in a variety of autoimmune diseases, therefore, it couldn’t diagnosis the disease of UC as A specific indicators. pANCA joint Hcy ulcerative colitis, the negative predictive value was significantly increased, both the negative can be used as index of exclusion of UC.
