Objective: Study the effect of recombinant human B-type natriuretic peptide on renal function in the patients suffered from heart failure combined with renal insufficiency thus investigate the important value of the renal function on prognosis of patients with heart failure. Methods: Retrospective analyze 110 heart failure patient with renal insufficiency hospitalized in the cardiac intensive care unit of the first affiliated hospital of xinjiang medical university from January 2014 to October 2015. According to medications, these patients divided into routine treatment group and rhBNP group. routine treatment group was given routine treatment, while rhBNP group was received routine treatment plus rhBNP with The maintenance dose intravenous infusion 0.0075Ug/(kg ?min), 72 hours of continuous use. Observe the clinical symptoms, signs, cardiac function(NYHA classification), serum creatinine, blood urea nitrogen, glomerular filtration rate, 24 hours urine, NT-proBNP, left ventricular ejection fraction(LVEF), left ventricular end-diastolic volume(LVEDD), left ventricular end-systolic volume(LVESD) of two Two groups before and after treatment and mortality, rehospitalization rate, recurrence of heart failure and renal function were followed up. Results: After treatment The rh BNP group was obviously improved in cardiac function when compared with the routine treatmen group The effective rate in the rhBNP group was 89.23%while that of the routine treatmen group was68.89%. serum creatinine, blood urea nitrogen, NT-proBNP, left ventricular end-diastolic volume(LVEDD), left ventricular end-systolic volume(LVESD) in rhBNP group was significantly lower than that in routine treatmen group(P<0.05, P<0.01), and glomerular filtration rate, 24 hours urine, left ventricular ejection fraction(LVEF), in rhBNP group was significantly higher than those in routine treatmen group(P<0.05) after treatment. the follow-up result of two groups, 13(20.96%) had recurred heart failure, 5patients(8.05%) died, 11(17.74%) were rehospitalized, 23(37.1%) had restored renal fanction In rhBNP group, ande 17 patients(39.53%) had recurred heart failure, 13(30.23%) died, 18(41.86%) were rehospitalized, and8(18.6%) had restored renal fanction in routine treatmen group. Conclusion: rhBNP in treatment of heart failure patient with renal insufficiency can more effective Iy improve renal function and protect renal function and delay the progression of heartfai Iure. rhBNP can significantly reduce the hospitalization rates, mortality, recurrence of heart failure rate, of heart failure patient with renal insufficiency and improve the prognosis.
