Construction Of Public Welfare Evaluation Index System And Its Evaluation In Public Hospitals

Medical and health service is an important part of "people’s livelihood", and it is the key foundation of social harmony and stability. Public hospitals are the main providers of basic medical services. The reform of public hospitals is not only related to the country’s financial capacity and economic development, but also closely related to the vital interests of the people. The reform of public hospitals is the focus and difficulty in the reform of the medical system. Maintaining the public welfare of their providing medical services has always been a concern. With the planned economy transiting to the market economy, the public welfare of hospitals has experienced from "approval" to "weaken" and then to "dilution". And now it’s facing unprecedented challenges. In the new healthcare reform, all the major policies emphasize on the public welfare of medical service and public hospitals as a guiding ideology. At the same time, solving the "difficult and expensive" issue of medical treatment is more and more urgent. In 2015, the comprehensive reform of county level public hospitals implemented nationwide. In 2016, the pilot cities of urban public hospitals widened to 200. The returning of public welfare has a long way to go.From the reviewing of the recent literature and policy, the author found that there was still an important precondition of gap:how to evaluate the public welfare of hospitals scientifically and objectively? What kind of measure tools should be used? These problems do not seem to have a consensus.Therefore, using the public goods theory, welfare economics theory and system theory, combing with the Delphi expert consultation method, this study tries to construct the evaluation index system of public welfare. Also there are 13 sample tertiary public hospitals which are representative picked to apply to the evaluation index system. Then this study analyses the problems of lacking public welfare, which appears during the evaluating process. Finally from specific to general, this study put forward the suggestions for returning to the public welfare, including three aspects: building system guarantee, improving internal mechanism and perfecting external environment. The objective of this study is to make reference for the government making reasonable policies, right supervising and managing, effectively promoting the public hospitals to public welfare, as well as for the public hospitals achieving their public welfare, for people choosing the proper hospital.
