Objective Urinary incontinence is a common disease of women, and stress urinary incontinence(stress urinary incontinence SUI) is the most common type. SUI mainly was caused by the pelvic floor breakage and dysfunction. It may appear at different stages of age and its incidence is showing a clear upward trend. The patient’s quality of life and physical and mental health has seriously damaged, affecting the daily work of most women patients, sex, social exchange activities, even leading to strong psychological pressure and neuropsychiatric disorders on patients. SUI has become the world of social health problems, causing widespread concern in the medical profession. Currently SUI has been diagnosed by clinical symptoms and laboratory tests. Due to factors of these ways were larger and laboratory diagnosis process was complex, easily leading tomisdiagnosis. The ALLP of urodynamic indicator is now a better method about the types of SUI, but it also has many deficiencies. This study aimed to explore the Valsalva maneuver under the two-dimensional, three-dimensional ultrasound indicators and female stress urinary incontinence severity relevance for providing accurate information on types of SUI.Methods Forty-nine normal female have been checked pelvic ultrasound, and forty-nine patients diagnosed as stress urinary incontinence(SUI) by clinic and laboratory have been examined through urodynamics and pelvic ultrasound. According to the three levels assessed by the International Advisory Committee of incontinence score, Ⅰ grade score ≤7 points,Ⅱ 8 points ≤grade score <14 points, Ⅲ 14 points ≤ grade score ≤ 21 minutes, we select important indicators related to SUI,including abdominal leak point pressure(ALPP) of urodynamics, parameters of twodimensional, three-dimensional ultrasound under Valsalva maneuver(bladder neck mobility, urinary bladder posterior horn, pubic area ofpelvic diaphragm hiatus(A), anteroposterior diameter(M), transverse diameter(N) and visceral muscles thickness(T1, T2)). Between the two groups(the experimental group and the control group) were compared using independent samples t-test, among the same grou-p( the experimental group) with varying degrees of using single factor analysis of variance(One-Way ANOVA). And a study on the correlation between these indicators and theseverity of SUI.Results 1. In the resting state, bladder neck mouths and urinary bladder posterior horns of SUI group and the control group were contrasted to show no change.There were no statistically significant difference between the two groups(P>0.05).Bladder neck mobility and urinary bladder posterior horns under Valsalva maneuver SUI group significantly bigger than the control group, the differences were statistically significant(P<0.05). 2. Three-dimensional pelvic ultrasonography basic indicators ofthe control group and SUI groups were contrasted in the resting state and under the Valsalva maneuver by independent samples t-test analysis. The parameters of two-dimensional, threedimensional ultrasound under Valsalva maneuver(bladder neck mobility, urinary bladder posterior horn, pubic area ofpelvic diaphragm hiatus(A), anteroposteriordiameter(M), transverse diameter(N) and visceral muscles thickness were sensitive on diagnosis of SUI(P<0.05). 3、Of relevant research leak point pressure and SUI severity,the difference was statistically significant(P<0.05);In these indicators of pelvic ultrasound,bladder neck mobility,urinary bladder posterior horn,pubic area A,and muscle thickness T1 are associated with severity of SUI under Valsalva maneuver(P<0.05),the differences were statistically significant.Conclusion 1. The parameters of two-dimensional, three-dimensional ultrasound under Valsalva maneuver(bladder neck mobility, urinary bladder posterior horn, pubic area ofpelvic diaphragm hiatus(A), anteroposterior diameter(M), transverse diameter(N) and visceral muscles thickness were sensitive on diagnosis of SUI 2. In these indicators of pelvic ultrasound,bladder neck mobility,urinary bladder posterior horn,pubic area A,and muscle thickness T1 are associated with severity of SUI under Valsalva maneuver.
