Objective:To improve the understanding of Spontaneous ruptured hepatocellular carcinoma (SRHC).Try to find the high risk population and analyze the treatment and prognosis of SRHC.Methods:From August 2009 to August 2015,55 patients of SRHC are treated in the Department of general surgery of Shandong provincial hospital affiliated to Shandong University. Among the 55 cases of patients, we can get 38 patients whose cases datas are complete through follow-up phone calls, letters and so on.We attempts to summarize susceptible factors of this disease by analysing the medical history,clinical characteristics and laboratory examination.Through the analysis of the clinical data and survival time of the patients, the factors influencing the prognosis of the patients were obtained.Results:In the total 38 patients, male patients are 31 cases, accounting for 81.6%; 33 cases of patients with liver cirrhosis,accounting for 86.8%;37 cases of HBsAg (+), accounting for 94.8%. Tumor volume exceeds 5cm in 29 cases, accounting for 76.3%; Patients with a history of alcohol are 25,accounting for 65.8%; In 29 patients,tumors located in the right lobe of the liver, accounting for 76.3%; Tumors protrude from the surface of the liver> 1cm are 22 cases, accounting for 57.9%. The 1 years,3 years, and 5 years survival rates are 55.2%、 10.5% and 5.3%,respectively. Single factor analysis in the hemoglobin level, Child-Pugh classification, treatment, prothrombin time, albumin and the level of total bilirubin are the relevant factors affecting the prognosis of patients. In multivariate analysis, serum albumin level and bilirubin level are the independent factors influencing the prognosis of patients.Conclusions:The risk factors of Spontaneous ruptured hepatocellular carcinoma are the populations with the following factors:middle-aged men, combined history of alcohol and liver cirrhosis history, tumors locating in the right lobe of the liver, the volumes exceeding 5cm,protruding the surface of the liver than lcm. Rupture of hepatocellular carcinoma is not a late stage of liver cancer.Hemoglobin level, Child-Pugh classification, choice of treatment methods, albumin level and total cholesterol levels are the risk factors of the prognosis of patients.The level of albumin and bilirubin are the independent factors influencing the prognosis of patients.
