The Effect Evalation Of Compound Danshen Dripping Pill In LDASO Patients After Lower-Limb Endovascular Interventions

ObjectiveAs the population ages, patients with diabetes surge. One of the most common chronic complications of diabetes is lower extremity arteriosclerosis. Patients with diabetic lower extremity arteriosclerosis obliterans were prone to foot ulcers, which in extreme cases led to amputation. Amputation not only affects the quality of life for a patient, but also carries social and economic burdens. A preferred alternative to treatment for many patients is percutaneous transluminal angioplasty. This treatment improves the blood circulation of diabetic lower extremity arteriosclerosis. Unfortunately in some cases the blood vessels of the postoperative can occur in-stent restenosis. The following study observed the effects of compound danshen dripping pill for intervention in patients with postoperative inflammation, high coagulation state, clinical symptoms, improvement of feet and of clinical outcomes and explored the intervention in patients with diabetic lower limb arteriosclerosis occlusion of restenosis after intervention, in hopes to provide effective prevention and control of restenosis after interventional methods.MethodTwenty-eight patients were assigned randomly to two groups, fourteen cases in each group. The two groups were given the same regular basic treatment. When blood glucose and pressure were stable, patients underwent percutaneous transluminal angioplasty. Three days before percutaneous, the experimental group patients took clopidogrel 75 mg once daily every morning on an empty stomach as well as compound danshen dripping pill,15 grain,3 times a day. The control group patients took clopidogrel,75 mg, once daily in every morning on an empty stomach for 24 weeks. Throughout this time the levels of Fg, CRP in postoperative 12 hours and the level of ABI, TcP02 after 24 weeks were compared between patients. The two groups were compared by the following points:rate of improvement of postoperative clinical symptoms, improvement of diabetic foot ulcers, rate of restenosis, and rate of amputation.ResuItsTwenty-five patients completed the clinical study. Thirteen were in the experimental group, while twelve were in the control group. Twelve hours after percutaneous transluminal angioplasty the levels of Fg and CRP of the two groups were relatively higher than 12 hours before the surgery with statistical difference (P<0.05). This increase was more obvious in the control group than the experimental group again with a statistical difference(P< 0. 05). After 24 weeks, the levels of ABI and TcP02 of the two groups improved with statistical difference (P<0.05). Notably the experimental group improved significantly compared with the control group (P<0.05).24 weeks after surgery, the clinical curative effectence rate was 84.62% and 41.67% for experimental and control groups, respectively, again there was a statistical difference of (P< 0.05). Additionally after percutaneous transluminal angioplasty, patients of both the experimental and control group of the diabetic foot ulcers Wagner hierarchical situation were improved. As a remark, there was no statistical difference (P>0.05). Moreover, one patient of the experimental group and two patients in control group exhibited blood vessel stenosis. From the control group one case required amputation of a toe. In consideration of the observed effects, vascular restenosis rate and amputation rate between the two groups were without statistical differencet(P>0.05).ConclusionTwenty-five patients completed the clinical study with follow-ups up to 24 weeks later. This study found compound danshen dripping pill did not significantly reduce the restenosis rate of diabetic lower extremity arteriosclerosis obliterans patients. Although it was found that treatment of compound danshen dripping pill combined with clopidogrel exhibited a therapeutic effect. This combination decreased the levels of Fg and CRP 12 hours after surgery while improving levels of ABI and TcP02 up to after 24 weeks, compound danshen dripping pill could be used to improve clinical symptoms of patients as well as preventing future blood vessel stenosis.
