ObjectiveThe purpose of this study was to investigate the defecation function of patients after first months,3 months and 6 months after operation in the patients with rectal cancer in Guangdong Province Traditional Chinese Medical Hospital. And the distribution characteristics of TCM Constitution of patients. To analyze the relationship between defecation function and TCM constitution type in patients with rectal cancer after anus preserving operation, so as to determine the high risk population and guide the individual treatment of the specific population.MethodsThis study used a questionnaire survey method, a total of 70 patients with rectal cancer in Guangdong Province Traditional Chinese Medical Hospital were collected. Prior to surgery of TCM physique evaluation. In the postoperative 1,3 and 6 months survey the frequency of defecation, fecal incontinence score, defecate not sense, defecation urgency, straining feeling of anus, anal rectum pain etc. defecation. The results were collected using SPSS19.0 statistical software for analysis. The number of stool frequency, stool symptoms score and other measurement data, using the paired t test and related sample rank sum test was analyzed. Using Fisher’s exact probability method has no precise analysis of postoperative stool frequency increased, there is no fecal symptom count data.Result1. Defecation function after anus preserving operation for rectal cancer:Abnormal defecation rate is very high, up to 98.6% in January,3 months after the operation, there are 95.7% abnormal stool,6 months after the operation, there are 90% patients with abnormal defecation. The postoperative 1 month of stool frequency increased (85.7%), abnormal stool (80%), defecation urgency sense (92.9%), stool not sense a higher incidence of (77.1%), fecal incontinence (8.6%), anal straining feeling (8.6%) and anorectal pain sense (8.6%) incidence rate is low. There was no change in the incidence of fecal incontinence and anal rectal pain after 3 months of operation. The incidence of defecation pain was slightly increased, and the number of other symptoms decreased.6 months after surgery, compared with before, in addition to defecate feeling no obvious changes, the number of patients with other symptoms has decreased significantly. After resection of rectal cancer the prevalence of fecal incontinence is not high. Compared with 1 months after surgery, most patients with fecal incontinence were reduced, but not completely disappeared in 3 months. The symptoms disappeared 3 months after operation, and the symptoms disappeared 6 months after operation. On the other hand, after resection of rectal cancer 1 month and 6 month defecation score, stool score, scores of fecal incontinence, defecation urgency sense score, endless sense of defecation score and anorectal pain sense score differences were statistically significant (P< 0.05). Sense of defecation score, there was no significant difference between the anal straining feeling score (P>0.05).2.70 cases of rectal cancer patients with TCM Constitution distribution is as follows:Yang deficiency (28.6%), Qi deficiency (17.1%), damp heat constitution (15.7%), Qi Stagnation (8%), special intrinsic quality (7%), flat and quality (5%), phlegm (3%), yin deficiency (2%), blood stasis (2%).3.70 cases of anus preserving operation for rectal cancer after 1,3 and 6 months of TCM constitution type in stool frequency increased defecation and endless sense of distribution difference has statistical significance (P< 0.05) and postoperative 1 month and 3 months of TCM Constitution Types in defecation urgency sense distribution difference with statistical significance (P< 0.05). Postoperative January TCM constitution type distribution differences in stool characters with statistical significance (P< 0.05). ConclusionThe research results show that the incidence of abnormal defecation function in patients with rectal cancer is very high.1 months after the operation,90% patients had abnormal defecation in 6 patients after 98.6% months. Among them, the number of bowel movements increased, the abnormal defecation, the sense of urgency, defecation, anal rectal pain and fecal incontinence will gradually improve and even disappear after the operation. A few patients with anal straining feeling will disappear, but most of the patients are not obvious. Defecation was not significantly improved in the second half of the year. Yang Xuzhi, Qi deficiency, damp heat is the susceptibility of colorectal cancer. Patients with Yang deficiency, Qi deficiency and damp heat constitution are more likely to increase defecation frequency, defecation and urgency.
