A Clinical Research Of 139 Patients With Behcet’s Disease

Objective:To analyze and summarize the clinical features of Behcet’s disease(BD), in order to provide some references for the clinical diagnosis and treatment of the disease.Methods:We retrospective analyzed the clinical material of 139 patients with BD, who hospitalized in the first affiliated hospital of Guangxi medical university hospital during January 2004 to October 2015. We had summarized the clinical features of all the patients. According to their gender、digestive system involvement and ocular involvement, all of the 139 patients were divided into sixth groups:the female group, the male group, the Intestinal BD group, Non-intestinal BD group, the ocular BD group, Non-ocular BD group. A comparison between each group would be done, include the clinical feature, general laboratory test results, treatment effect and prognosis. Data processing using SPSS 16.0 software, count data used ratio described and compared used the chi square test, measurement data used (mean±SD) or (median± four quantile interval) described, and compared used T test or Mann Whitney U test. Multivariate analysis used logistic regression analysis. All the results were statistically significant with P<0.05.Results:1.139 case of BD patients, male 85 cases, female 54 cases, male to female ratio was 1.57:1. All of the 139 patients, the average age is (30.09± 13.66), the mean disease duration is (4.59±5.25) years. BD could affect all the systems, in which recurrent oral ulcers are the most common clinical manifestations and initial presentation. Acupuncture-positive rate was 79.69%. There are 70 cases of BD patients with elevated ESR,91 cases with elevated CRP. BD pathological mucosa chronic inflammatory changes is the most common, followed by non-specific inflammation of small blood vessels and leukocytoclastic vasculitis.2. In male group and female group, the prevalence of ophthalmia difference was statistically significant(P<0.05); the rest of the clinical manifestations, the difference was not statistically significant. Male and female patients with BD compared to the results of laboratory tests, hemoglobin, IgG, IgM levels were significantly(P<0.05). Male BD patients prognosis is poor.3. The intestinal Behcet has 22 cases, male to female ratio was 1.44:1, the average age is (32.68±14.73), mean disease duration is (4.67±7.22) years. Abdominal pain is the most common gastrointestinal intestinal BD performance (54.55%), and is the most common first manifestation of the digestive tract (45.45%). Lesions involved the entire digestive tract, which the ileocecal junction was the most common site. Endoscopic performance was mainly for multiple small ulcers and deep huge isolated ulcers. Intestinal BD patients compared with non-intestinal BD patients, were more associated with conjunctivitis and less associated with uveitis, the difference was statistically significant (P<0.05). Hemoglobin levels were lower than normal in intestinal BD group.4.48 patients were associated with Ocular, accounting for 34.53%,the average age of Ocular was (31.65±11.55),93.75% for the eyes affected, 76.34% subject to varying degrees of vision impairment, blindness rate was 14.58%. The ocular BD can affect various structures within the eye, the full uveitis is the most common (36.5%). More men than women, were more likely to be associated with uveitis, all uveitis, corneal deposits, posterior synechia, cataract and vitreous opacities, the difference was statistically significant (P< 0.05). In addition, male patients with poor prognosis. The ocular involvement BD group has fewer genital ulcers, impetigo, and internal organ involvement. Through logistic regression analysis, we found that genital ulcer, impetigo may be a protective factor of eye of BD, but male may be a risk factor for eye BD.5.Most patients, who had used corticosteroids and immunosupp-ressive, had varying degrees of improvement. Of the 139 BD patients,4 died, which with digestive tract involvement.Conclusion:1. Behcet’s disease could occur in All ages, which 15-45 was the most common age, and it was more common in male.2.The intestinal Behcet’s disease mainly damaged the ileocecal, had less associated with uveitis.3. Ocular Behcet’s disease was more common in men, uveitis was the most common eye damage, which has a high blindness rate and a poor prognosis.4.Genital ulcer, impetigo may be a protective factor of ophthalmic Behcet’s disease, men may be a risk factor for ophthalmic Behcet’s disease.
