Objectives:To study the expression of interleukin-17A, interleukin-4 and inte rferon-gamma in nasal polyps, and investigated the machanism of Thl/Th2/Thl7 cells in the pathogenesis of CRSwNPs.Methods:82 specimens from chronic rhinosinusitis with nasal polyps in patients and 15 specimens of middle turbinate from patients with deviation of nasal septum were collected. According to the results of hematoxylin and eosin staining, nasal polyps tissues were classified into four types:eosinophils type (A type), neutrophils type (B type), noneosinophils nonneutrophils type (C type), both eosinophils and neutrophils type (D type). Randomly selected 15 cases from four types of nasal polyps and examined interferon-gamma, interleukin-4 and interleukin-17A for immunohistochemistry in nasal polyps and 15 specimens of middle turbinate in normal control. All the data are performed by SPSS 19.0.Results:1.The chronic rhinosinusitis with nasal polyps patients were classified into four groups:infiltration of eosinophilc type was found in 31.70%(26/82) of CRSwNPs patients and infiltration of neutrophils type was found in 18.82%(16/82) of CRSwNPs patients, infiltration of noneosinophils and nonneutrophils type was found in 48.24% (38/82) of CRSwNPs patients, and coinfiltration of esinophils and neutrophils type was found in 2.44%(2/82) of CRSwNPs patients.2.The results of immunohistochemical staining showed that the percentage of interleukin-17 A, interleukin-4 and interferon-gamma positive cells were significantly different in nasal polyps group and control group (P<0.01).3.The expression of IL-17A was significantly greater in type B than in type A, type C and control group (P<0.01).4.The expression of IL-4 was significantly greater in type A than in type B, type C and control group (P<0.01).5.The expression of IFN-gamma was significantly greater in type B and type C than in type A and control group (P<0.01).Conclusions:1.Interleukin-17A, interleukin-4 and interferon-gamma are key cytokines in the pathogenesis of nasal polyps.2.The eosinophilic CRSwNPs phenotype showed infiltration responses with predominant IL-4 and IL17A regulation.3. the neutrophils CRSwNPs phentype showed infiltration responses with predominantIFN-gama and IL-17A regulation.4.the noneosinophilic nonneutrophils groups showed infiltration responses with predominant IFN-gama regulation. Th1/Th2/Th17 play an important regulatory role in the inflammatory responses of CRSwNPs.
