The Relationship Between Premature Infant With Intrauterine /Extrauterine Growth Retardation And Orexin Or Leptin

Objective:To measure the levels of orexin、leptin in the preterm infant at their first day and at their correcting gestational age to 40 weeks,and to study the relationship between the orexin or leptin and the birth weigth,head circumference,to explore the orexin and leptin affecting neonatal growth and development.Methods:From 2015-4 to 2015-12 70 healthy neonates of Children’s hospital of Soochow University were selected and informed consent for their families,The preterm infants(40) were devided into two groups based on the relationship between gestational age and weight:the first day:the small for gestational age(SGA) and theappropriate for gestational age(AGA); the correcting gestational age to 40 weeks:the extrauterine growth retardation(EUGR) and the normal,and with full term infants(30) for the control group.Measure the neonatal birth weight, head circumference in the first day and when premature correcting gestational age to 40 weeks.The peripheral blood were collected,orexin and leptin were measured by enzyme-linked immunosorbent assay(ELISA).Results:1.The result of orexin level in the neonate’s first day:The result of orexin level in the full term infant was(7.80 ± 3.61)ng/ml,and(11.39 ± 5.47)ng/ml in the preterm infant,they had significantdifference(P<0.05).The result of orexin level in the SGA preterm infant was(14.14 ± 4.47)ng/ml,and(9.91 ± 5.45)ng/ml in the AGA preterm infant,they had significant difference(P<0.05).2.The correlation between the orexin level in the neonate’s first dayand weight,head circumference:There existed negative correlation between the orexin level and weight(r=-0.419,P<0.05),head circumference(r=-0.372,P<0.05).There existed negative correlation between the orexin level and weight(r=-0.529, P<0. 01),head circumference(r=-0.670,P<0.01).3.The preterm infants’ level of orexin at their correcting gestational age to 40 weeks:The result of orexin level in the EUGR preterm infant was(8.20 ± 3.44)ng/ml,and(5.15 ± 2.95)ng / ml in the normal preterm infant, they had significant difference(P<0.01).4.The result of leptin level in the neonate’s first day:The result of leptin level in the full term infant was(10.97 ± 4.74)ng/ml,and(6.84 ± 3.66)ng/ml in the preterm infant,they had significantdifference(P<0.05).The result of leptin level in the SGA preterm infant was(4.88 ± 2.01)ng/ml,and(7.90 ± 3.94)ng/ml in the AGA preterm infant,they had significant difference(P<0.01).5.The correlation between the leptin level in the neonate’s first dayand weight,head circumference:There existed postive correlation between the leptin level and weight(r=0.614,P<0.01),head circumference(r=0.362,P<0.05).There existed postive correlation between the leptin level and weight(r=0.463, P<0. 01),head circumference(r=0.441,P<0.01).6.The preterm infants’ level of leptin at their correcting gestational age to 40 weeks:The result of leptin level in the EUGR preterm infant was(8.75 ± 3.27)ng/ml,and(12.93 ± 2.48)ng / ml in the normal preterm infant, they had significant difference(P<0.05).7.The relationship between the leptin level and orexin level:The levels of orexin and leptin had no obvious relativity in the fullterm infant(r=-0.361,P=0.06).The levels of orexin and leptin had no obvious relativity in the preterm infant(r=-0.265,P=0.098)。Conclusions:1 There existed negative correlations between the orexin level and weight、head circumference in the full term infant and the preterm infant.There existed positive correlations between the leptin and weight、head circumference in the full term infant and the preterm infant.Prompting:orexin and leptin can response fetal nutritional status.2 The level of orexin of the EUGR is higher than the normal,but the level of leptin is lower,prompting:for the growth of fetus and the newborn,orexin may have the promoting role,leptin may have the inhibitory role.3 Between full term infant and the preterm,the orexin and leptin have no obvious relation.
