ObjectiveThe tension-free vaginal tape-obturator is minimally invasive procedure for the treatment of female Stress urinary incontinence with minimal risk of lower urinary tract (LUT) injury, maximizing the reproducibility of the tape’s insertion, and ensuring minimal tissue dissection. The study is to evaluate the clinical effect of tension free vaginal tape-obturator (TVT-O) in the treatment of stress urinary incontinence (SUI).Material and MethodsA total of 20 patients with SUI are selected for surgery with Gynecare TVT Obturator system (Tension-free support for incontinence) from May 2013 to February 2015 in the department of obstetrics and gynecology in the second affiliated hospital of Soochow university. Pelvic floor reconstruction with Avaulta synthetic support system or hysterectomy is done simultaneously in some of the patients with different degrees of pelvic organ prolapse, by a single experienced surgeon. We compared the intra-operative and postoperative conditions to describe the objective response of tension free vaginal tape-obturator (TVT-O). All patients are requested to complete the Urinary Distress Inventory (UDI-6) and Incontinence Impact Questionnaire (IIQ-7) as part of their pre- and postoperative assessments to subjectively evaluate the quality of life of the patient. Written informed consent was obtained from each subject.ResultThe data showed that there is a significant improvement in the quality of life of patients with stress urinary incontinence (SUI) after the tension free vaginal tape obturator (TVT-O) surgery. Urinary Distress Inventory (UDI-6) and Incontinence Impact Questionnaire (IIQ-7) administered before operation and at 3 months,6 months and 1 year after operation showed a statistically significant difference(p<0.05) between pre and post-operation.ConclusionTension free vaginal tape-obturator system is a minimally invasive procedure that has excellent results in the treatment of stress urinary incontinence with a low rate of complications. The (TVT-O) system has significantly improved the quality of life of patients. Currently it can be considered as the gold standard surgical treatment for stress urinary incontinence.
