| [Objective]At present there are various methods and technologies for estimating cerabral ischemic stages, but the method for estimating it with quantity is lack. It is through the way of choosing the most optimal parameters maps from CBF, CBV, TTP, MTT, TTD parameters maps of CTP parameters maps of acute and chronic cerebral ischemic patients to classify standardly and to calculate volomes of different kinds of cerabral ischemic stages to provide a more accurate quantitative criteria and to establish a new estimating method.[Method]1. All lesions of 58 acute and chronic patients with cerebrovascular disease were put into two parts in accordance with left and right brain and a sum of all of lesions was 116.2. According to the time from the onset time of disease to the time of receiving CTP operation, lesions were put into acute responsible lesions, no-acute lesions, chronic perfusion deficiency lesions; according to the number of lesions, lesions were put into single lesions and multiple lesions; according to the position of lesions, lesions were put into subcortical lesions and cortical and subcortical lesions. Original maps were processed by computer software, TTD parameter maps are choosen from CBF, CBV, TTP, MTT, TTD paremeter maps to be matched and be calculated, finally the volumes of core infarct regions, severe ischemic regions and relative ischemic regions are obtained.3. By the way of comparing core infarct regions and severe ischemic regions caculated by this method, the sensibility and specificity of different colours of TTD parameters maps reflecting different ischemic stages are explored.[Result]1. A sum of all of lesions was 116. The number of acute responsible regions is 35 and the number of no-acute regions is 35 and the number of chronic perfusion deficiency lesions is 46;the number of single lesions is 50 and the number of multiple lesions is 66;the number of subcortical lesions is 48 and the number of cortical and subcortical lesions is 58.2. The average volume of core infarct regions of acute responsible regions is 19.74±36.19ml and the average volume of severe ischemic regions of acute responsible regions is 23.44±15.26ml and the average volume of relative ischemic regions of acute responsible regions is 116.66±64.61ml; the average volume of core infarct regions of no-acute regions is 6.11±4.84ml and the average volume of severe ischemic regions of no-acute regions is 23.41±14.46ml and the average volume of relative ischemic regions of no-acute regions is 111.04±56.63ml; the average volume of core infarct regions of chronic perfusion deficiency lesions is 3.44±1.18ml and the average volume of severe ischemic regions of chronic perfusion deficiency lesions is 15.71±9.18ml and the average volume of relative ischemic regions of chronic perfusion deficiency lesions is 101.74±56.29ml;the average volume of core infarct regions of single regions is 18.64±39.18ml and the average volume of severe ischemic regions of single regions is 39.74±26.11ml and the average volume of relative ischemic regions of single regions is 105.71±77.19ml;the average volume of core infarct regions of multiple lesions is 10.64+20.68ml and the average volume of severe ischemic regions of multiple lesions is 38.67± 29.17ml and the average volume of relative ischemic regions of multiple lesions is 118.73 ±68.16ml;the average volume of core infarct regions of subcortical lesions is 22.61±32.11ml and the average volume of severe ischemic regions of subcortical lesions is 36.17±29.58ml and the average volume of relative ischemic regions of subcortical lesions is 109.71±59.14ml; the average volume of core infarct regions of cortical and subcortical lesions is 23.19±38.15ml and the average volume of severe ischemic regions of cortical and subcortical lesions is 38.26±31.21ml and the average volume of relative ischemic regions of cortical and subcortical lesions is 111.74±67.26ml.3. The sensibility and the specificity of the red region of TTD parameter map reflecting the core infarct regions of acute responsible regions is 57.75% and 93.70%, and reflecting the core infarct regions of no-acute lesions is 43.57% and 90.57%, and reflecting the core infarct regions of chronic perfusion deficiency lesions is 40.34% and 90.34%, and reflecting the core infarct regions of single lesions is 67.23% and 92.20%, and reflecting the core infarct regions of multiple regions is 33.27% and 89.27%, and reflecting the core infarct regions of subcortical lesions is 50.74% and 91.74%, and reflecting the core infarct regions of cortical and subcortical lesions is 55.60% and 89.60%, and reflecting the core infarct regions of all lesions is 52.68% and 88.95%, The sensibility and the specificity of the yellow region of TTD parameter map reflecting the severe ischemic regions of acute responsible regions is 50.40% and 78.70%, and reflecting the severe ischemic regions of no-acute lesions is 45.37% and 80.69%, and reflecting the severe ischemic regions of chronic perfusion deficiency lesions is 43.41% and 82.33%, and reflecting the severe ischemic regions of single lesions is 60.31% and 80.84%, and reflecting the severe ischemic regions of multiple regions is 40.53% and 73.72%, and reflecting the severe ischemic regions of subcortical lesions is 45.51% and 78.42%, and reflecting the severe ischemic regions of cortical and subcortical lesions is 46.90% and 81.70%, and reflecting the severe ischemic regions of all regions is 45.54% and 78.98%. The sensibility and the specificity of the green region of TTD parameter map reflecting the relative ischemic regions of all regions are both 100%。[Conclusion]The volume of core infarct region, severe ischemic region and relative ischemic region of acute and chronic cerebral ischemic stages are calculated by the maps matching method and can be used as more accurate and immediate evidences for evaluating infarct region and ischemic penumbra, clinical thrombolysis and treatment. In totally the specificity of the red regions of TTD parameter maps reflecting core infarct region of all regions and the specificity of the yellow regions of TTD parameter map reflecting the severe ischemic region of all regions are high,especially in single regions. The sensibility of the red regions of TTD parameter maps reflecting core infarct region of all regions and the sensibility of the yellow regions of TTD parameter map reflecting the severe ischemic region of all regions are lower than specificity. |
