| ObjectiveTo understand the inflammatory response characteristics in HIV/TB co-infection patients, and try to explore the value of CRP, D-dimer and TB-IGRA in the clinical diagnosis of HIV/TB co-infection.MethodsUsing immunoturbidimetry and ELISA, CRP, D-dimer and TB-IGRA were measured in 136 patients with HIV/AIDS, also clinical data were collected.Result136 patients were enrolled,22 of them were diagnosis as HIV/TB co-infection. And compared with the non HIV/TB co-infection group, the HIV/TB co-infection group with a lower CD4+account (t=2.03, P< 0.05), higher CRP, D-dimer and TB-IGRA level (with the Mann-Whitney U value of 293.5,487 and 354.5, all p< 0.001). Diagnostic test shows that CRP, D- dimer and TB-IGRA have the better effect of co-infect judgment. (AUC>0.8).but signal CRP:sensitivity 0.773, specificity 0.737; D-dimer sensitivity 0.500, specificity0.991; TB-IGRA:sensitivity 0.773, specificity0.904; As two or more contain two positive:sensitivity 0.636, specificity0.965; All three are all positive:sensitivity0.455, specificity 1.000.Suggest specific degree is better,sensitivity is poor.ConclusionHIV-TB co-infection patients showed stronger inflammatory response, CRP, D-dimer and TB-IGRA synthetic assessment has better clinical value, also it may have potential value in early diagnosis of HIV/TB co-infection.But still can’t avoid missed diagnosis in the early diagnosis of TB co-infection still need to find more valuable markers or methods. |
