ObjectiveWe collected the clinical characteristics and clinical summation of pyogenic liver abscess to judge the progress of the disease early. To evaluate the correlation of all laboratory indexes in pyogenic liver abscess and screen for key indicators to evaluate disease prognosis.MethodsFrom January 2014 to June 2015, there were 60 patients with pyogenic liver abscess in Sir Run Run Shaw Hospital, School of Medicine, Zhejiang University, were included in the study. The patients were divided into two groups:recovered group and unrecovered group, and their clinical manifestations, laboratory tests, image examination, pathological examination and drug sensitive results were analyzed. Both univariate and multivariate analyses were performed by using Logistic regression to build forecasting model. ROC curve was drawn to calculate sensitivity and specificity to validate the model.ResultThe result of 60 cases were involved in this study. Middle-aged and elderly people with aged 60.6 ± 14.5 were easily suffered from pyogenic liver abscess and male patients outnumbered female patients. Fever (98.3%, peak temperature 39.1±1 chills (58.3%) and abdominal pain (48.3%) were the main manifestations. Many patients had different underlying diseases:diabetes (40%), hypertension (41.7%), biliary disease (40%), pulmonary infection (21.67%), multiple renal cysts (50%).Klebsiella pneumoniae was the most common pathogen (82.1%) of pyogenic liver abscess and was sensitive to the third generation cephalosporins and carbapenemes. It was found that age and prothrombin time were independent risk factors for predicting prognosis of pyogenic liver abscess (p< 0.05). We integrate the two factors into pyogenic live abscess prognosis value P, and the AUROC, based on the prediction probability, was 0.915. Its sensitivity and specificity was 77.8% and 97.6%, respectively.ConclusionAge and prothrombin time were independent risk factors for predicting prognosis of pyogenic liver abscess. Pyogenic liver abscess prognosis value combined with clinical manifestations, laboratory tests, image examination and pathological examination had better ability to judge the progress of the disease.
