Objectives:Through the detection of rheumatic valvular heart disease (RHD) patients with left and right atrial tissue collagen content and distribution and SK2 protein and gene expression of chronic atrial fibrillation (AF) in patients with sinus rhythm in patients with atrial tissue collagen and SK2 table of and differences, and chronic atrial fibrillation in patients with left and right atrial tissue between collagen and SK2 expression and differences, understanding atrial fibrosis, clear atrial remodeling.Methods:Selection during to August 2015-April 2016, the rheumatic valvular heart disease in our hospital and accept 31 patients with valve replacement surgery, extracorporeal circulation turn before the flow were obtained from left and right atrial appendage tissue as the object of study, including 17 cases of atrial fibrillation patients, sinus rhythm group with 14 cases. The two groups of left and right atrial tissue was used conventional Masson staining for histological examination. At the same time the application of automatic image analysis system for the determination of collagen volume fraction, using hydroxyproline content determination method for the determination of left and right atrial tissue collagen content, enzyme linked immunosorbent assay (ELISA), WB detection and comparison of left and right atrial tissue of SK2 protein expression, RT-PCR was performed to detect and comparison of left and right atrial tissue in SK2 mRNA level, income data were statistically analyzed.Results:1. General results showed that:the long duration of AF group were, on average, rheumatic heart disease, patients with sinus rhythm group compared with statistical significance (P< 0.05), atrial fibrillation in patients with LAD and CTR was significantly greater than that of sinus rhythm group (P< 0.05); and between the AF group and sinus rhythm group between age, sex, weight, rad, LVEDd, LVEF index difference no statistical significance (P> 0.05).2.the results of collagen distribution and content showed that:Masson staining showed that:under the microscope, the collagen fibers were blue, and the myocardial tissue was red. Compared with patients in sinus rhythm group and atrial fibrillation in patients with left and right atrial tissue collagen volume fraction were significantly increased (P< 0.05); atrial fibrillation group, left, right atrial tissue collagen volume fraction between difference no statistical significance (P> 0.05). The content of hydroxyproline assay results showed that:compared with the sinus rhythm group, the content of collagen in atrial fibrillation patients left and right atrium were significantly increased (P< 0.05); atrial fibrillation group, left, right atrial collagen content difference no statistical significance (P> 0.05).3.The results show that the ELISA method was used to detect the real AF atrial tissue of SK2 protein concentration:3.96±0.52ng/ml and sinus rhythm group atrial tissue of SK2 protein concentration:1.18±0.15ng/ml, SK2 protein expression between the two groups, the difference is statistically significant (P< 0.01).4.Western Blot assay showed that:the protein bands of left and right atrial tissue in the atrial fibrillation group were significantly deeper than that in the left and right atrial tissues. Compared with the sinus rhythm group, the relative expression of SK2 protein in the atrial fibrillation group was significantly increased (P<0.01), and there was no significant difference in the relative expression of SK2 protein between the left and right atrial tissues (P>0.05).5. The RT-PCR results showed that:compared with the sinus rhythm group of left and right atrial tissue, atrial fibrillation group, left and right atrial tissue kcnn2 mRNA relative expression amount and the difference was statistically significant (P< 0.05); atrial fibrillation group, left, right atrial KCNN2 mRNA relative expression amount, the difference had no statistical significance (P>0.05).Conclusions:1 atrial fibrillation patients with left and right atrial tissue extracellular matrix, such as the structure of the obvious pathological changes:a large number of collagen fibers in the myocardium, separated, wrapped around the heart tissue, high degree of fibrosis. The content and distribution of collagen in atrial tissue increased with the occurrence of atrial fibrosis and atrial fibrillation.2. rheumatic heart disease with atrial fibrillation patients with atrial tissue, SK2 protein expression increased, but SK2 gene expression decreased and atrial fibrillation with left and right atrial tissue SK2 gene and protein expression had no significant difference, suggesting that SK2 expression changes may be one of the important molecular mechanism in the process of the development of the occurrence of rheumatic heart valve disease in patients with atrial fibrillation, and provides a theoretical basis for the further research and prevention and treatment of atrial fibrillation.
