ObjectiveThrough this research analyze whether cohort study is suitable for the study of combine traditional Chinese and western medicine in the treatment of liver cirrhosis. And through retrospective cases analyze whether cohort study can reflect the combine traditional Chinese and western medicine treatment the curative effect of hepatitis b cirrhosis of the liver. At the same time analysis the clinical cohort study on existing problems and put forward the corresponding countermeasure, provides the suggestion for the future prospective studies.MethodsOf cirrhosis of the liver by relevant clinical literature and cohort study on related literature were analyzed, discuss whether the cohort study is suitable for the study of combining traditional Chinese and western medicine in the treatment of liver cirrhosis, and analyze common problems in the research of queue and put forward the corresponding countermeasures. In addition, on January 1,2004 to June 30,2015 in Guangdong province hospital of university and Zhuhai hospital outpatient or hospitalization diagnosed with hepatitis b cirrhosis compensatory phase of the medical records of 129 patients were retrospectively analyzed. Statistical analysis of data with statistical software in order to make clear the curative effect of combining traditional Chinese and western medicine treatment of hepatitis b cirrhosis of the liver. And discuss in this cohort study on existing problems and the reasons and puts forward the method to solve the problem.ResultsThrough literature research, clear cohort study with a sample group, long observation when input is relatively small, the advantages of various clinical data collected at the same time, has some advantages in chronic diseases research. In addition, cohort studies allow individualized treatment and outcome evaluation index is simple and easy to measure, especially suitable for clinical research of traditional Chinese medicine.This study include 129 cases. Differences of groups of gender, age and follow-up period have no statistically significant.On liver function aspects, Western medicine, combining Chinese and western medicine group before and after treatment the ALT value difference was statistically significant. The doctor of traditional Chinese medicine group there was no statistically significant difference ALT values before and after the treatment. ALT after treatment there was no statistically significant difference between the groups. AST values before and after the combination of Chinese and western medicine treatment group difference was statistically significant. There was no statistically significant difference western medicine and Chinese medicine group. AST after treatment there was no statistically significant difference between the groups. The doctor of traditional Chinese medicine and combining Chinese and western medicine group propagated value difference was statistically significant between before and after treatment, there was no statistically significant difference the western medicine group. After treatment was statistically significant difference between the propagated groups, combine traditional Chinese and western medicine group and western medicine group is higher than Chinese medicine group. Three groups of TBIL differences before and after the treatment, and there were no statistically significant difference between the TBIL groups after treatment. Western medicine group of PT value difference was statistically significant before and after the treatment, there was no statistically significant difference in TCM and combine traditional Chinese and western medicine group. There was no statistically significant difference between the PT group after treatment.Virus, western medicine, combining Chinese and western medicine group before and after the treatment of HBV-DNA value difference was statistically significant,there was no statistically significant difference in TCM group. After the treatment of HBV-DNA was statistically significant difference between the groups. Combine traditional Chinese and western medicine group and western medicine group of HBV-DNA was lower than those of group of traditional Chinese medicine. Combine traditional Chinese and western medicine group of HBsAg difference was statistically significant before and after the treatment, the doctor of traditional Chinese medicine and western medicine group no statistical difference. After treatment was statistically significant difference between the HBsAg groups, group was lower than those of western medicine group and combine traditional Chinese and western medicine of traditional Chinese medicine. Three groups of HBeAg differences before and after the treatment, and no statistically significant difference were observed in the HBeAg after treatment.APRI values, western medicine and traditional Chinese and western medicine combined with group difference was statistically significant before and after the treatment, there was no statistically significant difference in TCM group. There was no statistically significant difference between the groups after treatment.Routine blood and B ultrasonic integral, three groups before and after treatment the WBC, RBC, HB, PLT, B ultrasonic integral differences and differences between the treatment group were not significant.Clinical outcomes,2 cases of cirrhosis of the liver decompensation period of western medicine group,1 case of liver cancer of western medicine group and 3 cases of combine traditional Chinese and western medicine group,1 case death of traditional Chinese medicine group.This clinical retrospective cohort study, family history of an unknown proportion as 23.26%, drinking history of of an unknown proportion is 19.38%, the proportion of people infected with hepatitis b in time is unknown at 20.16%. An unknown situation had no statistical difference between the groups. This study average follow-up time was 31.63 months, always lost to follow-up rate was 10.9%, lost to follow-up cases in group differences without statistical significance.86.05% of data in this case study is not complete.ConclusionAfter 129 cases of hepatitis b patients with liver cirrhosis compensatory period clinical retrospective cohort studies and related literature analysis clear cohort study in the study of combining traditional Chinese and western medicine in the treatment of cirrhosis of the liver has the feasibility and application prospect.Through a retrospective cohort study can be clearly combine traditional Chinese and western medicine treatment the curative effect of hepatitis b cirrhosis of the liver. Combine traditional Chinese and western medicine therapy and western medicine treatment in anti-inflammatory and inhibiting viral replication is pure traditional Chinese medicine treatment. Combine traditional Chinese and western medicine treatment in the aspect of improving albumin is pure Chinese medicine or western medicine.But this retrospective cohort study also inevitably exist lost to follow-up, bias, the problem of lack of information and a large difference in the number of groups. May cause a certain impact on the results. The credibility of the results decrease. In the prospective study, we should pay more attention to the study of the actual situation of the cohort study.
