ObjectiveThrough the analysis of the therapeutic effect of Professor Huang Yuhua's combination of traditional Chinese and Western medicine in the treatment of viral hepatitis related cirrhosis with ascites and the analysis of common Chinese medicine,to explore the composition, efficacy, sexual taste, meridian tropism and the compatibility of common Chinese medicine of Professor Huang's commonly used prescriptions,so as to provide the treatment ideas and methods for the patients with decompensated cirrhosis with ascites.MethodsFrom October 2017 to October 2019,the patients were hospitalized in the hepatology Department of huayuanshan hospital,Hubei Hospital of traditional Chinese medicine,88 patients were diagnosed as decompensated ascites due to chronic hepatitis B or C by Professor Huang Yuhua.Record the patient's general situation, blood routine before and after medication, coagulation function, liver function, renal function,abdominalB-ultrasound or abdominal CT and other medical records and prescriptions of traditional Chinese medicine, and establish an Excel database.IBM SPSS was used for patients' gender, age,TCM syndrome type, tongue image, pulse image, frequency of drug use, high frequency drug efficacy, four Qi,five flavor and meridian return 20.0 frequency analysis, paired design t-test analysis of patients' weight, blood routine, coagulation function, liver function, renal function, abdominal B-ultrasound or abdominal CT before and after medication, and visual analysis of commonly used drugs.Results1.Generally,55 cases were male, accounting for 64.71%,30 cases were female, accounting for 35.29%,the ratio of male to female was 1.93:1;the age of onset was 33-70 years old,the average age was 65.85 ± 21.47 years old;the highest proportion of syndrome type was hot and humid water stop syndrome.2.Comparison of comprehensive clinical efficacy: 19 cases(22.35%)were clinically relieved,36 cases(42.35%)were significantly improved,28 cases were effective, accounting for32.94%;2 cases were ineffective, accounting for 2.35%;the total effective rate was 97.65%.After treatment, the body weight and abdominal circumference decreased, the difference was statistically significant(P < 0.00);24-hour urine volume increased, the difference was statistically significant(P < 0.00);Comparison of blood routine indexes before and after treatment:before and after treatment, the percentage of leukocyte, neutrophil and hypersensitive C reactive protein in patients with spontaneous bacterial peritonitis decreased compared with those beforetreatment, the difference was statistically significant(P < 0.05),the platelet increased compared with those before treatment, the difference was statistically significant(P < 0.05)There was no significant difference in the percentage of leukocytes, neutrophils, erythrocytes, hemoglobin, platelets, and high-sensitivity C-reactive protein before and after treatment in patients without spontaneous bacterial peritonitis(P > 0.05).After treatment, prothrombin time, international standardized ratio, alanine aminotransferase, aspartate aminotransferase,r-Glutamyltransferase,alkaline phosphatase, total bilirubin, direct bilirubin, indirect bilirubin, serum creatinine, urea nitrogen, serum cystatin C were significantly lower than before treatment, the difference was statistically significant(P < 0.05),percentage activity of prothrombin The activity of albumin and cholinesterase increased significantly(P < 0.05).3.The study included 85 cases and 85 prescriptions,involving82 traditional Chinese medicines, with a total of 888 prescriptions.Secondly, according to the pharmacodynamic classification, the drugs for clearing heat, promoting water and swelling, nourishing yin and regulating Qi are the main drugs;according to the four Qi classification, the cold drugs are the most frequently used;according to the five flavor classification, the bitter and sweet drugs are the most frequently used;according to the meridian classification, the spleen, lung, liver, stomach and kidney are the main drugs;it is found that the commonly used drugs are Poria Atractylodes Rhizoma alismatis,Poria Atractylodes Rhizoma Atractylodes macrocephala,Poria Atractylodes Rhizoma Yinchenhao,Poria Atractylodes Rhizoma Polyporus, etc.;the corePrescription: Poria cocos,Atractylodes macrocephala,Alisma orientalis, wormwood wormwood, big belly skin, turtle shell,Polyporus, licorice,Astragalus.ConclusionProfessor Huang Yuhua can effectively control infection, improve the function of detoxification and synthesis of liver, and prevent the occurrence of serious complications such as hepatorenal syndrome. For this disease,Professor Huang's basic treatment is invigorating the spleen and replenishing qi,clearing away heat and promoting dampness, and softening and dispersing the structure.
