Objective To explore the clinical significance of serologic examination (pepsinogens, gastrin-17 level and Helicobacter pylori IgG antibody) for screening atrophic gastritis and precancerous lesions in early gastric cancer high-risk groups in the Xixiangtang community of Nanning, Guangxi Zhuang Autonomous Region, in order to improve the diagnosis of early gastric cancer.Methods 347 patients were recruited who assessed for upper gastrointestinal cancer high-risk groups by the cancer risk assessment questionnaire,the serum pepsinogen I and II were tested by Chemiluminescence Microparticle Immuno Assay(CMIA),the serum gastrin-17 and helicobacter pylori (Hp)-IgG were tested by enzyme-linked immunosorbent assay (ELISA), according to the results of pathologic examination, grouped as follows:normal group, chronic non-atrophic gastritis, chronic gastric-antrum atrophic gastritis group, chronic gastric-body atrophic gastritis, intestinal metaplasia group and dysplasia group. Combined with its serum PGI, PGII,PGR (PGI and PGII ratio), G-17 and Hp-IgG and pathological results to statistical analysis.Results Chronic gastric-body atrophic gastritis group (9.8±2.4ng/mL) PGII levels than the normal (5.2±3.5ng/mL), chronic non-atrophic gastritis(7±4.1ng/mL) and gastric antrum atrophic gastritis group(7.3±4.3ng/mL) increased (p<0.05); PGR level of all the other groups were lower than the normal group (8.3±1.9ng/mL) (p<0.05);Chronic atrophic gastritis (gastric-body) group were lowest (3.9±1.4ng/mL) (p<0.05).About G-17 level chronic atrophic gastritis (gastric-antrum) group (22.1±20.6ng/L) were lowest. Compare the positive group with the negative group of Hp-IgG, PGII(6.9±4.4 vs 6.9±4.4)ng/mL go up, PGR (5.7±2.1 vs 6.9±2.4) ng/mL go down (p< 0.05).Conclusions:The go up of PGII level and down of PGR in the gastric cancer high-risk groups suggested that there might be chronic gastric-body atrophic gastritis, the drop of G-17 suggested there might be gastric antrum atrophic gastritis, the gastric mucosa H. pylori infected shows easier to occur atrophy lesions than noninfected.Combined use of serum PG, G-17 and Hp-IgG shows important clinical significance for screening atrophic gastric gastic mucosa lesions.
