To evaluate the effect and safety of Baduanjin training on cognitive function in old patient with mild cognitive impairment systematically. To provide evidence for its popularization and application.MethodsA total of 87 eligible subjects were randomly allocated into Baduanjin group (28), brisk walking group (30) or Health education group (29). The Health education group received the expain of health education related to cognitive function every two months. Baduanjin and brisk walking group received baduanjin exercise and brisk walking respectively for 24 weeks with 3 times per week and 60 minutes per time based on the health education. The main outcome was gloabal cognitive function, using the Montreal Cognitive Assessment Scale (MoCA) to evaluate. The secondary outcomes included memory and quality of life. The Wechsler Adult Memory Scale (including instantaneous memory, short-term memory and long-term memory) and Auditory Vocabulary Learning Test (including instant recall, delayed recall and delay recognition) were applied to assess the memory. The MOS item Short From Health Survey was evaluated to the quality of life. Primary and secondary outcomes was measured at baseline and end of 24-week intervention. The gloabal cognitive function and memory was measured every 2 months during intervention. The pre-protocal (PP) and intention-to-treat (ITT) analysis were used to compared the difference among three groups with one-way ANOVA or Nonparametric test. The multiple interpolation was apllied to ITT analysis and we interpolated one time. Statistical analyses will be performed using SPSS 22.0. The statistical significance is defined as two-sided Pvalue of< 0.05.Results1. Gloabal cognitive function:PP analysis showed that after 6 months intervention, the MoCA scores in Baduanjin group was higher than the health education group (P=0.009) and it was also higher than the Brisk walking group with no statistical difference (P=0.071). The ITT analysis found that there was no significant difference among 3 groups. Global cognitive function in Baduanjin group was higher than health education group (P=0.015) and Brisk walking group (P=0.002) after adjusted gender and baseline using mixed linear model analysis. However, there was no significant deference between Brisk walking and Health education group (P>0.05).2. The exploratory analysis on memory as follows, (1) Instantaneous memory:there was no significant significance on instantaneous memory among three groups adopting the PP and ITT analysis. However, after adjusted the mixed factors, the instantaneous memory in Baduanjin group was significantly higher than Health education group (P=0.012) and Brisk walking group (P=0.010), and there was still no significant difference in Brisk walking and Health education group (P=0.874). (2) Short-term memory:three methods of analysis shown that there was significant difference in the score of picture recall among three groups and the score in Baduanjin group was higher than Health education and Brisk walking group (P<0.05). PP and mixed model analysis showed the score of image recognition in Baduanjin group is significantly higher than the other two groups (P<0.05). Three methods of analysis also shown that understanding memory in Baduanjin group was significantly higher compared with Health education group (P<0.05). The image regeneration (P<0.001) and tactile memory (P=0.008) appeared significant difference between Baduanjin group and Brisk walking group after adjusted the mixed factors, and the Baduanjin group was significantly higher than Brisk walking group. (3) Long-term memory:three methods of analysis shown the score of mind B in Baduanjin group was significantly higher than the other two groups (P<0.05). ITT and mixed model analysis results showed that the score of mind A in Baduanjin group was significantly higher than Health education group (P<0.05). (4) Instant recall:compared with Health education and Brisk walking group, all the analysis showed significant defference in the second and third recall in Baduanjin group (P<0.05). PP and ITT analysis showed that the score of the first recall in Baduanjin group was higher than Health education group and Brisk walking group(P<0.05). (5) Delayed recall and Delayed recognition:the score of 15 minutes delayed recall and delayed recognition in Baduanjin group were significant higher than Health education group(P<0.05). There was no significant defference between Health education group and Brisk walking group in delayed recall and delayed recognition.3. Quality of life:ITT analysis results showed that the body pain have statistically difference (P=0.035<0.05) and the score of Baduanjin and Brisk walking group were higher than the Health education group.ConclusionSix-month baduanjin training has potential improvement on global cognitive function and memory in old adults with mild cognitive impairment.
