The Correlation Among Pain Beliefs,uncertainty In Illness And Quality Of Life Of Patients With Lumbar Disc Herniation

Abstract/Summary:
Objectives To investigate the pain belief, uncertainty in illness and quality of life in patients with lumbar disc herniation. To explore the relationship among pain belief, uncertainty in illness and quality of life in patients with lumbar disc herniation, and to analyze the influencing factors of quality of life of patients with lumbar disc herniation.Then to giving evidence for nursing staff to seek intervention measures to improve the quality of life of patients.Methods A total number of 236 patients with lumbar disc herniation were recruited from the 163 Hospital of PLA, the Hunan Province People’s Hospital between January 2015 and July 2015. The research instruments included General Information Questionnaire, The Pain Beliefs and Perceptions Inventory, Roland-Morris disability questionnaire, Mishel’s Uncertainty in illness Scale-Adult and The MOS 36-item Short Form Health Survey.(SF-36). All data input was used by Software Epidata 3.1and SPSS18.0 was used for statistical analysis. Statistical methods included descriptive statistical analysis, Mann-Whitney U test,Kruskal-Wallis H test,correlation analysis and multiple linear step regression analysis.Results(1)236 questionnaires were distributed, and 222 questionnaires were valid, the valid recovery rate was 94.07%. Among the patients with lumbar disc herniation, the average age of was(51.41 ± 14.78), with 9(4.1%) mild pain, 19(8.6%) moderate pain, 148(66.6%)heavy pain and 46(20.7%) severe pain.(2)The median and quartile of quality of life in patients with lumbar disc herniation were 32.91 and 22.30 respectively. The score of physiological function and the emotional function was the lowest, and their median were 0.00.(3)The median total scores of pain beliefs in patients with lumbar disc herniation was 6.00, The median of belief of Pain as Permanent score was0.75 and the quartile was 2.00. The median of Beliefs about Pain as Mystery, Self-blame and Pain as Constant were 0.50, 0.33 and 0.00 respectively.(4)The total scores of uncertainty in illness of patients with lumbar disc herniation was(75.93 ± 11.55), the uncertainty dimension score was(48.33 ± 8.00) and complexity dimension score was(27.59 ± 4.63), The total scores of degree of disability in patients was(17.45± 5.37).(5)One-way ANOVA showed there were significant differences in the total scores of quality of life among patients with age, education levels,places of residence, occupation, gross joint income monthly, type of medical expenses payment, type of medical treatment, pain level and other diseases. Multiple linear regression analysis results showed that the regression model was predicated by places of residence, type of medical expenses payment, pain level, other diseases and degree of disability.(6)Spearman correlation analysis showed the Pain as Mystery scores,Pain as Constant scores, Pain as Permanent scores, Self-blame scores, the total scores of pain beliefs and the total scores of degree of disability had negative correlation with quality of life(r=-0.246,r=-0.303,r=-0.550,r=-0.313,r=-0.535,r=-0.676,P<0.01); the uncertainty dimension and the total scores of uncertainty in illness had negative correlation with quality of life(r=-0.157,r=-0.246,P<0.05); the total scores of pain beliefs had positive correlation with uncertainty in illness and degree of disability(r=0.370,r=0.442,P<0.01).Conclusions(1)The median of quality of life in patients with lumbar disc herniation was 32.91, lower than the general population.(2)The quality of life in patients with lumbar disc herniation is influenced by places of residence, type of medical expenses payment,pain level and degree of disability. In those factors, degree of disability impacted it significantly.(3)The pain beliefs and uncertainty in illness in patients with lumbar disc herniation had negative correlation with quality of life. Pain beliefs had positive correlation with uncertainty, suggesting that the morenegative pain beliefs had by the patients with lumbar disc herniation, the higher uncertainty in illness, the worse the quality of life.
