| ObjectiveTo explore the clinical prognosis of patients with urinary system obstruction secondary to operation of gastric cancer.MethodsPatients with urinary system obstruction secondary to gastrectomy (RO resection) in our department between April 2009 and June 2014 are retrospectively reviewed. Every patient after the surgery came to our department regularly to reexamine the abdominal CT. Diagnosis of urinary system obstruction is based on the maximal ureteral diameter greater than 7 mm under CT scan. Kaplan-Meier and log-rank test were used to analyze overall survival(OS).Results25 patients with urinary system obstruction were included. The median follow-up time was 44 months (range 18-80 months).12 patients (48%) presented with urinary system obstruction at a median time of 17months(range of 6 to 42 months) after the gastrectomy, meanwhile no other evidence of tumor recurrence was found; Until the last follow-up,10 patients suffered tumor recurrence, and the remaining 2 patients showed no indication of tumor recurrence; The median time is 4 months (range 1 to 26 months) from the first diagnosis of urinary system obstruction to tumor recurrence. The other 13 patients (52%) presented with both urinary system obstruction and tumor recurrence, which occurred a median time of 14 months(range of 1 to 40 months).The median overall survival time of these 25 patients is 31 months(95%CI range 24.8-37.2 months). The overall survival (OS) at 1,2 and 5 years is 88%(95%CI 75.3%-100%),35% (95%CI13.6%-56.4%),5.8%(95%CI 0%-16.8%) respectively.ConclusionsThe prognosis of patients with urinary system obstruction secondary to gastrectomy are poor, which is a hint of tumor recurrence. This situation should be given high attention to. |
