Objective:Allergen immunotherapy (AIT) is presently the only treatment able to modify the natural history of airborne allergic disease. The efficacy of AIT relies on the administration of the right allergen(s) to each allergic patient. In vivo and in vitro routine allergic testing is carried out using natural extracts. These contain an array of proteins, some allergenic and some other not. Most allergenic proteins are species-specific, whereas some are phylogenetically highly conserved and show a high degree of similarity with homologous proteins present in distinct allergen sources. The latter allergenic proteins are responsible for cross-reactivity between distinct allergen sources, and the sensitization to these proteins represents a major diagnostic problem in respiratory allergy. The exceptional advances in molecular biology that have taken place during the last decades have led to detect, sequence, and produce in recombinant form a number of allergens, many of which are presently commercially available for the so-called component-resolved diagnosis (CRD). The objective of this article is to investigate the Der f 1 and Der f 2 specific IgE values in Chinese house dust mite-allergic patients, and provide clinical data for CRD.Methods:eighty patients were enrolled, Serum IgE reactivity to D. farinae, (Df) was measured by ImmunoCAP and Zheda Dixun Immunoblotting method. Serum IgE reactivity to Der f land Der f 2 was also measured in Df-allergic patients.Results:65 cases had positive D. farinae specific IgE in 80 cases by both detection ways, and the positive rate is 81.3%. Most Df positive patients were sensitized to Der p 1 (87.7% positive rate) and Der p 2 (90.8% positive rate), with both allergens together (97% positive rate) being largely responsible for the total anti-Df IgE response. The special IgE value of Der f 2 is significantly higher than that of Der f 1.Conclusion:IgE responses to Df and its two major allergen components were higher in children than in adults in this study. These results suggest that Der f 1 and Der f 2 are the dominant allergens in patients with Df allergy in Zhejiang. Significant different of the IgE level exists between Der f 1 and Der f 2, the latter one is higher than the former one. Children with higher IgE concentration of Der f 1 and Der f 2 than adults.
