[Objective]1. To investigate the value of lymphocyte immunotherapy in repeated implantation failure;2. To discuss the changes of lymphocyte immunophenotyping after lymphocyte immunotherapy for repeated implantation failure patients and the possible mechanism of lymphocyte immunotherapy.[Method]The 179 cases who are patients with repeated implantation failure from March 2015 to December 2015 of IVF-ET admitted to the Department of reproductive medicine of Second Affiliated Hospital of Kunming Medical University for assisted reproductive techniques were retrospectively analyzed. Divide them into fresh and frozen embryo transfer cycle and each cycle into observation groups and control groups by whether lymphocyte immunotherapy are used before embryo transplantation. The clinical pregnancy rate, implantation rate of observation groups and control groups after applying lymphocyte immunotherapy were investigated. The changes of lymphocyte immunophenotyping after lymphocyte immunotherapy for observation groups were discussed.[Results]1. Fresh cycle:A total of 108 cases includes 51 observation cases and 57 control cases. The clinical pregnancy rate, implantation rate of observation groups (47.06%,24.17%) was significantly higher than that of control groups (22.81%,13.18%) and the difference was statistically significant (P<0.05), and due to the small number of abortion cases, the abortion rate did not undergo statistical comparison.2. Frozen cycle:A total of 71 cases includes 32 observation cases and 39 control cases. The clinical pregnancy rate, implantation rate of observation groups (46.88%,24.36%) was significantly higher than that of control groups (23.77%,12.89%) and the difference was statistically significant (P<0.05), and due to the small number of abortion cases, the abortion rate did not undergo statistical comparison.3. the changes of lymphocyte immunophenotyping after lymphocyte immunotherapy for observation groups:there was no obvious difference in the proportion of peripheral CD3+T cells, CD4+T cells, CD3+HLA-DR+T cells, CD3-CD19+B cells in the cases of observation groups before and after lymphocyte immunotherapy. The proportion of peripheral CD8+T cells was significantly increased, while the ratio of CD4+/CD8+ and the proportion of peripheral CD3"CD16+CD56+NK cells was decreased after lymphocyte immunotherapy (P<0.05).[conclusion]1. The lymphocyte immunotherapy can improve the outcomes of repeated implantation failure patients.2. The proportion of CD8+T cells, CD3’CD16+CD56+NK cells, the ratio of CD4+/CD8+ in peripheral blood is changed obviously after lymphocyte immunotherapy. The CD8+T cells are increased and the CD4+/CD8+, the CD3-CD16+CD56+ NK cells are decreased. The CD8+T cells might be beneficial for pregnancy maintenance. The lymphocyte immunophenotyping of CD8+ T cells and CD3-CD16+CD56+ NK cells could be used as one of the important indicators for the therapeutic effect of lymphocyte immunotherapy.
