Objective:To explore the ability to identify the patients’ risk by clinical nurses,and preliminary construct the evaluation index of nurses for the ability of identify the risk of patients,to make clear and evaluate nursing risk identification ability for hospital nursing management,and provide a reference of different levels of education and training.Methods:By using the methods of literature analysis,expert interview to insure the basic principles,framework and contents of specific indicatiors;using the Delphi method to establish the index system of dimensions and content,using analytic hierarchy process(AHP)to determine the weight of index;using the index system of preliminary construction compiled into a questionnaire that for the clinical nurses to self-assessment there ability to identify patients’ risk. The pre survey about 200 clinical nurses form Third Grade Class A General Hospital of Shanxi and examine its reliability and validity.On the basis of good reliability and validity, 1161 nurses in Shanxi Province 4 Third Grade Class A General Hospitals were measured, and analysis the single factor of the data.Results:1.Preliminary construction of the index system for nurses to identify the risk ability of patients: Invited 28 nursing specialists to participate in two rounds of expert consultation,the recovery rate of two rounds of questionnaires were 100%.Expert consultation results show that the judgment(Ca) was 0.89, familiar with the indexes(Cs)were 0.81, the degree of authority(Cr) was 0.85.The coefficient of variation(CV) of two rounds of expert consultation was <0.25, and the harmony coefficient(W)was >0.3,shows that the coordination of expert opinion is more consistent.Finally, the 3primary indexes and weights are established,the nurses’ ability to control the risk of the patients(0.0904), the nurses’ risk perception ability(0.5559) and the nurses’ risk analysis ability(0.3537) respectively and 21 indexes of two level.2.The reliability and validity of the index system of risk ability of nurses:Reliability:the overall Cronbach’s alpha coefficient was 0.956, the split half reliability was0.938;Validity: confirmatory factor analysis results of χ2/df=1.82, approximate root mean square error(RMSEA) was 0.062, standard fit index(NFI) was 0.97, not the standard exponential fitting(NNFI) was 0.99, root mean square residual(RMR) was 0.022,comparative fit index(CFI) was 0.99, incremental fit index(IFI) was 0.99, the relative fit index(RFI) was 0.97.According to the literature, as long as the multiple criteria, the model is a good fit. It can be considered that the model fit is good and the model can be used for reference. In summary, the questionnaire has good reliability and validity.3.The application of index system about nurses identify patients risk ability : Through the single factor analysis, the different sections of the category, length of service, marital status, professional title, professional attitude,gender of nurse, children, whether for team and with teachers and the nine factors are influencing factors of risk ability of nurses to identify patients.Obtained by stepwise regression analysis, and on the ability of nurses’ risk identification has significant effect of a total of five, the sequence of professional attitude,professional title, whether for the team, having children and whether the teachers.Conclusion:1.To establish the index system for nurses to identify the risk ability of the patients,including 3 first level indicators and two 21 level indicators;2.Index system has good reliability and validity.3.Factors that affect the ability of nurses to identify risk factors are the different sections of the category, length of service, marital status, professional title, professional attitude,gender of nurse, children, whether for team and with teachers.The professional attitude, professional title, whether for the team, having children or not and whether the belt teachs a teacher can have a significant impact on risk ability of nurses to identify patients.
