The Changement Of Small Airway Function After Treatment In Adult Asthma Patients

Objective:To understand the reversibility of small airway function in adult patients with asthma; To study the relationship between small airway function and asthma control test; To investigate the evaluation role of small airway function on the asthma curative and prognostic judgement.Methods:184 patients were retrospectively analyzed by diagnoised as bronchial asthma initially according with the asthma diagnosis and grading standards of GINA. Detailed medical history, physical examination,filling in the asthma control test were registered.And then 76 patients were divided into groups A and group B by their airway function after bronchial provocation test/bronchodilation test before been treated. A groups(34patients): normal small airway function, B groups(42 patients): abnormal small airway function. After 3 months follow-up, compare the lung function、ACT scores and acute asthma attack between the two groups.Results:The airway function of two groups were improved after tretment, and the B group changed more significantly. But small airway of B group, like FEF50%、 FEF75% 、MMEF, failed to return to lower limit. The small airway of B group recovery ratio is41.94%. The small airway of A group recovery ratio is 71.43%. FEV1%Pred 、FVC%Pred、FEV1%FVC、FEF50% 、FEF75%、MMEF of two group had no statisticaldifference(P>0.05). FEV1%Pred、FVC%Pred、FEV1%FVC、FEF50%、FEF75%、MMEF of two group had statistical significant difference(P<0.01). ACT scores exist positive correlation with FEV1%Pred、FEF50%、FEF75%、MMEF. Group B had higher recurrent rate ratio than group A.Conclusion:Adult bronchial asthma patient were under control after normalizational treatment,but small airway of someone didn’t return to normal range. Abnormal small airway function often accompanied by large airway dysfunction, and the ratio was higher than normal small airway function.Small airway function had significant correlation with asthma control test. In combination with small airway function may be better to acess asthma control state. Small airway function of bronchial asthma is likely to be one of the factor to judge risk and indictor to assess prognosis.
