Objective: To determine whether the pulmonary emphysema index has a correlation with pulmonary complications after pulmonary lobectomy at high altitude.Methods: Select the patients who underwent pulmonary lobectomy with lung cancer from October 2014 to September 2015 at high altitude area. Fifty eligible patients were received preoperative MSCT(multi-slice spiral CT) examination.Inspection time should be within a week before surgery. With the help of computed tomography automatic processing software, we would got the emphysema index values(EI values) for analyzing three-dimensional QCT lung volume and lung density and setting the parameters.Depending on preoperative EI values,patients were divided into A, B, C, D four groups according interval EI values ≤10%, 10% <EI values ≤20%, 20% <EI values ≤30%, EI values>30%. All of patients undergo single lung resection.It needs to collect the postoperative clinical data related to pulmonary complications: blood gas analysis, imaging data, postoperative Sp O2,results of sputum culture, blood routine examination,etc.Results: Two people who do not meet the requirements of preoperative CT examination were excluded from this study.Another 48 patients underwent the operation successfully. According to the perioperative clinical observation and statistical analysis,we have found that patients’ preoperative pulmonary emphysema index have a positive correlation with postoperative respiratory complications(r=0.403,P= 0.005<0.01) and conversely the EI values(13.43±10.41)(%)have a negative correlation with postoperative mean pules oxyhemoglobin saturation values(91.72±3.06)(%)(r=-0.69,P=0.007<0.01)at high altitude region.Conclusion: 1,Combining with clinical observation and statistical results, the EI values have a clear correlation with pulmonary complications after pulmonary lobectomy,in particular,have closely correlation with postoperative pules oxyhemoglobin saturation values.2, EI is the one of CT pulmonary function criterion and has some predictive effects on pulmonary complications after pulmonary lobectomy.The EI might serve as one indicator for evaluating pulmonary function before pulmonary lobectomy.
