Objective:The study was aimed to construct the core competence evaluation index system of clinical nurse specialist,so as to provide a scientific evidence of the cultivation,assessment and evaluation for clinical nurse specialist.Methods:Used Delphi method to construct the core competence evaluation index system of clinical nure specialist,combined analytic hierarchy process to determine the weight of evaluation index system.Used Excel 2003, SPSSl3.0, AHP software for data processing and analysis.Results:1.Sort out the concept of clinical nurse specialist,clinical nurse specialist is expert nurse,who achieve the master’s or doctoral degree through the systematic study and practice, through accreditation,use their experience and theoretical knowledge, qualified for a variety of clinical practice,nursing expert counseling,nursing management,research and other work.2.Clear the scope of work of clinical nurse specialists,including direct clinical care,using critical thinking to provide care for those complex circumstances cases and heavy /moderate-risk patients;to provide counseling services for Patient/family,junior nurses,facilitate patient outcomes and nursing practice;to educate undergraduate and graduate nursing students,to led the development of special nursing education;to develop the relevant standards,norms,guides in specialist areas actively,to promote nursing practice,improve the working environment,evaluate and guidance services to improve the quality of care so as to protect the safety of care;to detect specialist clinical practice issues,and pointed out valuable research to explore,and preside over the implementation of research,to promote scientific research and applications.It can be seen the work of clinical nurse specialist is not only simple care operations,but also the integrated of nursing practice,education, administration and research.3. Through two round of expert consulting to select the indicators,construct the clinical nurse specialist core competence evaluation index system,including 5 primary level indicators,14 secondary indicators and 45 third-level indicators.4.The Positive coefficient of two round consultant were 0.57,0.85,28 experts proposed suggestions,the experts have a high enthusiasm.Judgment coefficient,familiar coefficient and authoritative coefficient of experts were 0.9,0.8,0.85,the experts have a high degree of expert authority.In the second round of consultation,the coordination coefficient of primary index and the two indicators were 0.283,0.185, experts recognized the extent of the good indicators.5.The weight of five primary level index from high to low order are: clinical practical ability(0.3184),consulting guidance ability(0.3184),professional development ability(0.1829) and clinical scientific research ability(0.1179),nursing management ability(0.0624).6.The evaluation system has good reliability and validity.All indicators Cronbach’s αcoefficient was 0.929, 5 dimensions Index Cronbach’s α coefficient between 0.735-0.862.the result have a high degree of expert authority, expert opinions were focused,have a better content validity.Conclusion:Through sort out the clinical nurse specialist, nurse specialist,nurse leader,specialist nurse and other related concepts, clarified the concept of clinical nursing specialists,based on the results of the research, constructed the clinical nurse specialist core competence evaluation index system, determined the weight of all levels, the result had a better reliability and validity,can provide a basis for the training, assessmenting and evaluating of clinical nurse specialist.Suggestions:1.Recently speed up the training of specialty nurse, gradually increase the number of the highly educated specialized nurse.2.Establish clinical nurse specialist positions when the health services and health care in the forefront of developing countries.3.Establish and improve the clinical nurse specialist management system,and provide care platform for clinical nurse specialist as soon as possible.Originality in this study:Based on hierarchical management of nurses,learn from the experience of clinical nurse specialists domestic and international,distinguish the concept of clinical nurse specialist and nurse specialist, nurse leader, nurse specialists, then clarify the concept,based on the concept and functions,construct the clinical nurse specialist core competency evaluation index system, make up the current shortage of clinical nurse specialist core competency research.Research deficiencies and prospects:Further improve the elements of clinical nurse specialist core competence,doing empirical research for the existing clinical nurse specialists in China.
