The Curative Analysis Of Small Incision Between Ribs Of Surgical Options For Excision Of Thymoma

Objective:(1).To evaluate the clinical application and value of small incision to remove earlythethymoma;(2).Analyse the influence of many factors on the prognosis of early thymoma resection;(3).Summarize the indication of the rib small incision to remove early thymomaindications. Methods:Select 45 case of people who had thymus removed from The hospital of Shanxi Clinical University during January 2006 to January 2015,it is divided into three groups, one is accepted surgical operation in the middle of chest,the other is in ribs small incision, the anoter way is called Video assisted thoracoscopic surgery.Each group has 15 cases.Comparing the surgical operation time, intraoperative blood loss, postoperative pain degree,length of hospital stay,the remission of the Myasthenia gravis,mortality of difference. Through analysing multi-factor analysis of thymoma operation method,we find the influential factors of Thymic tumor prognosis. Results:The SNK method shows that: operation time and postoperative hospital stay is different in small incision group, the Video assisted thoracoscopic surgery group and transsternal thymectomy group. Nonparametric test shows that: intraoperative blood loss and chest flow is very different in the three groups,the P<0.01. When α1=0.05,α2=0.1, Cox multifactor analysis shows that: the organization type is the important factor which affects the prognosis of time. When α1=0.2,α2=0.3, Cox multi-factor analysis shows that: operation method, tumor volume and the organization type are the factors which affects the prognosis of time. Conclusion:1,Small incision between ribs thymoma resection is feasible and safe.2,Cmopeting with transsternal thymectomy, it has less complications,quicker recoveryand a similar shorter postoperative hospitalization time.It has no evident differencewith transsternal thymectomy and Video assisted thoracoscopic surgery.3,When the early thymoma is not in the middle part, its diameter is less than 8cm,orthe people in the poor health and want the incision is secret,we can choose smallincision between ribs of surgical options or VATS.
