Objective: To compare the short-term clinical efficacy of the SuperPATH of minimally invasive total hip arthroplasty with posterolateral small incision total hip arthroplasty.Methods: From July 2014 to July 2015,80 patients treated with unilateral total hip arthroplasty(Double lower limbs length are less than or equal to 1cm)were included.All cases were randomly divided into the observation group which use SuperPATH as treatment and the control group which use posterolateral small incision as treatment,compared two groups of patients with operation, hospitalization time, Harris score and postoperative dislocation rate.Results: Two different surgical approaches were satisfied with the curative effect.Postoperative review of the hip joint radiographic outcomes showed that all cases were satisfied with the position of the joint prosthesis. Acetabular abduction angle: the observation group is about 41.03°±7.30°,the control group is 39.49°±5.52°. The two groups of bilateral offset values are less than or equal to 4mm, double lower limbs length are less than or equal to 5mm. To compare with the small incision group,there was no significant difference in the length of incision between the two groups(P>0.05),the SuperPath group is significantly decreased in intraoperative bleeding volume,postoperative drainage and hospitalization time(P < 0.05),but the operation time of the SuperPath group is longer than small incision group(P<0.05); Besides,in the first 24 hours after operation, the SuperPATH group's CPK and thigh circumference comparedwith preoperative values were significantly less than the small incision group, the differences were statistically significant((P<0.05). However, in first 24 hours after operation two groups' C- reactive protein(CRP) and erythrocyte sedimentation rate(ESR) compared with preoperative values, showed no significant difference(P>0.05);the SuperPATH group's Harris hip score was increased significantly than small incision group from 3 months and 6 months after surgey(P < 0.05), but till nine months there was no significant difference between two groups in Harris score(P > 0.05); the postoperative follow-up found no dislocation case of SuperPATH group while two cases of small incision group with early posterior dislocation of the hip after operation, but the two groups of patients with joint dislocation were analyzed, showed no significant difference(P > 0.05).Conclusion: SuperPATH total hip arthroplasty is the minimally invasive hip replacement surgery with small surgical incision, the less soft tissue injury through the muscle gap approach, less bleeding, quick postoperative joint functional recovery, the lower dislocation rate of postoperation, is worthy of recommendation.
