Objectives To investigate the characteristics, clinical effect factors and achievement of tibial plateau fractures of Schatzker V and VI and recommend the operation methods and surgical treatment choice.Methods From 2009 to 2013,96 patients of Schatzker V VI underwent internal fixation. HSS functional grading system and SF-36 rating system was used to assess the knee function, and the SPSS 17.0 was used to analysis. Evaluating the relationship between clinic prognosis and various influencing factors.Results 71 patients included in this study were followed up from 24 to 71 months with an average time of 50.2months.Bone union was achieved in all of them. According to HSS functional grading system, the excellent, good, fair and poor cases were 45,19,4 and 3. According to SF-36 health survey system, all patients were damaged in different severity, mainly in dimensions of PF, BP, GH, RP and VT.Conclusions Tibial plateau fractures of Schatzker V and VI should be treated according to the classification and CT scan to choose suitable approach and fix pattern.The complete fracture reduction, bone transplantation, stable internal fixation, good protection and recovery of the soft tissue and proper positive rehabilitation are all key factors to improve the therapeutic effects.But factors which make disadvantage to the prognosis are:(1) Angulation deformity is more than 9°. (2)Tibial plateau is 3.5mm wider than femur condylar or subluxation is more than 6mm when angulation deformity is less than 9°. (3) Deep infection.
