Objective To explore the etiology of sudden sensorineural hearing loss in children and to analysize the therapeutic effect and the factors related to prognosis of sudden sensorineural hearing loss in children.Methods A retrospective review of medical records of 49 chlidren(61 ears) with sudden sensorineural hearing loss who were hospitalized into our department from January 2008 to September 2015. Carry on the statistics with patient’s age, gender, initial time of therapy, the onset side of the ears, accompaning symptoms, treatment methods, hearing before and after the treatment, serum virology and genetic test results. They are grouped according to patient’s gender,age,degree of hearing loss,the pattern of audiogram,initial time of therapy,sick ear, accompanying sympotoms.Every one was given conservative comprehensive treatment.Analysize the therapeutic effect in different groups and through SPSS19.0 software chi-square test to compare the difference in therapeutic effect of each group.Finally,by making multiple logistic regression analysis method for statistically significant factors evaluates the correlation to prognosis.Results Among 49 patients(61 ears), 32 ears(65.3%) accompanied with tinnitus and 5 ears(7.9%) were concomitant vertigo.10 cases(24.5%) suffered from virus infection before diease;2 cases were due to head trauma;1 case was noise-related;38 patients for virology test of serum the positive rate of CMV IgM, suspected EBV Ig M were 0.08%,0.03%;9 cases for genetic test,2 of them had GJB2 gene 109G>A homozygous mutations.14 ears(23.0%) were mild and moderate hearing loss,14 ears(23.0%) were moderate-severe and severe hearing loss,and 33 ears(54.1%) were profound hearing loss.The proportion of 4 audiogram patterns:low frequency hearing loss,high frequency hearing loss,flat frequency hearing loss and total frequency hearing loss were 0.0%,11.5%,27.9%,60.7%.After treatment,the complete recovery rate was 13.1% and the overall recovery was 26.2%.The degree of hearing loss,audiogram pattern,age were significantly asscociated with therapeutic effect. However,making multiple logistic regression analysis showed that the prognosis was no correlation to the degree of hearing loss, audiogram pattern and age.Conclusion1. Etiologies of pediatric sudden sensorineural hearing loss include viral infection, hereditary factor,trauma, noise.Virus infection is likely the most common cause of pediatric sudden sensorineural hearing loss.2. Pediatric sudden sensorineural hearing loss is generally profound hearing loss,its most audiogram pattern is total frequency hearing loss,and its therapeutic effect is poor.3. The singer factor of analysis of influence on the prognosis of pediatric sudden sudden sensorineural hearing loss shows the milder hearing loss,the elderly children,the better treatment efficacy.The initial time of therapy,sex,bilateral or unilateral,concurrence of vertigo and tinnitus has no significant influence on therapeutic effect.4. Multiple logistic regression analysis indicates the prognosis has no significant correlation to the degree of hearing loss, audiogram pattern and age.
