ObjectiveThe purpose of this study was to explore the clinical curative effect of the expandable pedicle screw( EPS) combined conventional pedicle screw fixation for thoracolumbar burst fractures in the senile patients with osteoporosis.MethodFrom January 2009 to December 2011,27 patients with thoracolumbar burst fractures and osteoporosis were treated by EPS combined conventional pedicle screw fixation and followed up regularly to observe the improvement of the heights of anterior and posterior vertebral body、the Cobb angle、the preoperative and postoperative VAS score, and to evaluate the stability of the screw and the situation of the spinal fusion; The Beaujon-Lassale function scores was assessment at the last follow-up post-operatively.ResultAll patients were followed up for 18-31 months(average, 24 ± 2.7months), in which 10 patients were undergo internal fixator removal at18-24 months(average, 21.5 ± 2.2 months) post-operatively; At the pre-operation, one week postoperatively and final follow-up,the average anterior(posterior) height of the vertebral body, the average Cobb angle and the average VAS score were observed. All indices above were significantly improved, there was a significant difference of all indices between at the preoporation and on week postoperatively(P<0.05), but no significant difference between final follow-up and one week postoperatively(P>0.05).All patients were cured and instrumentations were not loose and broken.The Beaujon-Lassale function scores was 19.2 ± 2.7 at the last follow-up post-operatively. ConclusionThe posterior expandable pedicle screw combining conventional pedicle screw fixation in treatment of thoracolumbar burst fractures in senile patients with osteoporosis has the well clinical curative effect on promoting fracture healing and improving postoperative pain and reducing the kyphosis deformity.
