| Objective: Medical disputes is the manifestation of the deterioration of the doctor-patient relationship, the in-depth reforming of medical and health system did not make significant improvement to the current situation of the medical disputes, on the sustainable development of social stability and public health, resolve conflicts between doctors and patients play an extremely important role. Through investigation and analysis of Chongqing county hospital medical disputes, understand the status quo, both doctors and patients study the impact factors of medical disputes, put forward practical and feasible measures and suggestions.Methods: This study mainly uses the research methods of the literature analysis, research methods case studies, surveys and statistical analysis, text data collected from the domestic and foreign databases of CNKI, Pub Med database and other domestic, and relevant national policy document on medical disputes, data from the stratified sampling medical staff and patients of seven county hospitals in Chongqing by the questionnaire of doctor-patient relationship and medical disputes cognition, collected medical dispute case in one of county hospitals from 2011 to 2013 occurred in conduct consolidate, then summarize the relevant data, classification and statistical analysis, statistical analysis applications. Use the SPSS20.0 software, in order to make percentage description, chi-square test and multivariate Logistic regression analysis.Result:Current situation: The frequency of medical disputes of county hospitals in Chongqing increased every year, the top three causes of medical disputes reason is: the treatment results are not satisfactory, the high cost of medical treatment, doctor-patient communication difficulties; the way of dealing with the medical dispute is settled by negotiation between doctors and patients; when patients are most dissatisfied with medical treatment followed by long waiting times, high cost, registration is difficult; The evaluation of medical staff on the doctor-patient relationship is negative, a high degree of concern about the occurrence of medical disputes, the current assessment of the current medical environment is relatively low.Influence factors: Medical disputes occurred in middle-aged and elderly patients with the majority; no health insurance, paid for health care patients prone medical disputes; medical costs, the greater the probability of occurrence of medical disputes; surgery is the highest risk department of medical disputes. Medical disputes for high risk factors in the medical staff to men, surgery, high professional title, work load, the patient’s attitude is poor and choose to use the risk of medical technology.Conclusions: At present, the situation of medical disputes in county hospitals is deteriorating, and the influencing factors are varied.the lack of health personnel which represents grassroots medical institutions, medical personnel, weak awareness of health services, health risk prevention and other prominent enough, the conflict between economic and cultural level of patients in country and their demand for medical services, the strengthening of the concept of rights is obvious. Some suggestions are put forward: by the rational allocation of human resources in county hospitals, such as through government departments to increase investment in primary medical institutions, strengthening the construction of hospital management team; encourage hospitals and doctors to buy the medical liability insurance; train medical personnel medical service awareness, increase the medical service level of county hospitals, to improve medical personnel doctor-patient communication skills, strengthen risk prevention and control of medical awareness, develop the diversified ways of mediation, and bulid the people’s mediation mechanism of medical disputes in county hospitals. Effectively ease the tense doctor-patient relationship to the parties to bring the pressure to reduce the occurrence of medical disputes. |
